Dosage/Direction for Use
Adult : Topical Acne As 0.5-2% preparations: Apply to affected area 1-3 times/day. Hyperkeratotic and scaling skin conditions As 1.8-3% preparation: Apply to affected area 1-4 times/day. Warts; Callus As 12-40% plaster: Fit over the wart/callus for 48 hr, repeat 48 hrly as needed until wart/callus is removed (≤12 wk for warts or ≤14 days for calluses). As 5-17% preparation in collodion-like vehicle: Apply a small amount to cover wart/callus and allow to dry. Repeat 1-2 times/day until wart/callus is removed (≤12 wk for warts or ≤14 days for calluses). As 15% preparation in karaya gum-glycol plaster vehicle: Smoothen warts w/ emery board then place a drop of warm water prior to application. Apply the plaster in the evening and leave for at least 8 hr to be removed in the morning. Repeat 24 hrly, if necessary up to 12 wk.

Adult: As 0.5-2% preparation: Apply thinly to affected area 1-3 times daily, reduce to once daily or every other day if dryness or peeling occur.

Contraindications
Wart/callus removal preparations: Diabetes or impaired blood circulation. Use on moles, birthmarks and unusual warts w/ growing hair or warts on the face.
Special Precautions
Patient w/ impaired peripheral circulation or diabetes, significant peripheral neuropathy.
Adverse Reactions
Dermatitis, skin peeling, discomfort, irritation, dryness, ulceration or erosion; systemic effects including salicylate toxicity (e.g. nausea, vomiting, dizziness, lethargy, hyperpnoea, diarrhoea, and psychosis), and toxic inner ear damage.
Potentially Fatal: Rarely, serious hypersensitivity reactions (e.g. anaphylactic reaction).
Patient Counseling Information
Avoid use on broken skin, mouth, eyes, mucous membranes and anogenital region.
MonitoringParameters
Monitor for signs of salicylate toxicity. Sensitivity testing for 1st time users.
Drug Interactions
Increased risk of skin dryness and irritation w/ other topical acne preparations.
Action
Description: Salicylic acid has a potent keratolytic action and a slight antiseptic action when applied topically. It softens and destroys the stratum corneum by increasing endogenous hydration which causes the horny layer of the skin to swell, soften, and then desquamate. At high concentrations, salicylic acid has a caustic effect. It also possesses weak antifungal and antibacterial activity.
Onset: Treatment of warts: 1-2 wk.
Pharmacokinetics:
Absorption: Readily absorbed through the skin.
Distribution: Plasma protein binding: 50-80%, to albumin.
Excretion: Via urine [salicyluric acid (52%), salicylate glucuronides (42%) and salicylic acid (6%)].
