Carbamazepine
Indications/Uses
Epilepsy (except absence & myoclonic seizures). Acute mania. Maintenance for bipolar affective disorders to prevent or attenuate recurrence. Alcohol w/drawal syndrome. Trigeminal neuralgia, idiopathic glossopharyngeal neuralgia.
Dosage/Direction for Use
Adult 100-1,600 mg daily in divided doses depending on indication & severity of illness. Epilepsy Max: 2,000 mg daily in divided doses. Trigeminal neuralgia Max: 1,200 mg daily in divided doses. Childn 10-20 mg/kg daily in divided doses, >15 yr Max: 1,200 mg daily in divided doses, 6-15 yr Max: 1,000 mg daily in divided doses, up to 6 yr Max: 35 mg/kg daily in divided doses.
Administration
Should be taken with food: CR tab: Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity. AV block. History of bone marrow depression & hepatic porphyria. Concomitant use of MAOIs.
Special Precautions
Serious skin (eg, SJS/TEN), allergic & hypersensitivity reactions including drug rash w/ eosinophilia & systemic symptoms. History of cardiac damage & adverse haematological reactions, mixed seizures, hyponatremia, hypothyroidism. Suicidal ideation or behaviour. Increased risk of fall, fractures & other injuries. Initial & periodic complete blood count, liver function tests & urinalysis. Initial testing for patients +ve on HLA-B*1502 allele (eg, Chinese ancestry) & HLA-A*3101 allele (eg, Japanese, Caucasian, indigenous Americans, Hispanic, Southern Indians, Arabic ancestry). Monitor plasma levels. Women taking hormonal contraceptives. Avoid abrupt w/drawal. May affect ability to drive or operate machines. Liver disease; hepatic or renal damage. Women of childbearing potential should use effective contraception during treatment & for 2 wk after last dose. Pregnancy & lactation. Preexisting increased IOP, latent psychosis, confusion & agitation in elderly. Syr: Allergy to sorbitol & parahydroxybenzoates.
Adverse Reactions
Dizziness, ataxia, fatigue, drowsiness, allergic skin reactions, leukopenia, nausea, vomiting, increased γ-GT. Edema, fluid retention, increased wt, hyponatraemia & decreased blood osmolarity, headache, accommodation disorders, diplopia, liver enzyme elevations, thrombocytopenia, eosinophilia, dry mouth, increased blood alkaline phosphatase, allergic dermatitis, urticaria.
Drug Interactions
Concomitant use of CYP450 3A4 inducers or inhibitors. Hormonal contraceptives, alcohol.
