Timolol maleate
Indications/Uses
Reduction of elevated IOP in patients w/ ocular HTN, chronic open-angle glaucoma, aphakia & glaucoma, secondary glaucoma (some cases), narrow angles & history of spontaneous or iatrogenically-induced narrow-angle closure in the opposite eye in whom reduction of IOP is necessary.
Dosage/Direction for Use
1 drop in affected eye(s) once daily. Transfer from Timoptol to Timoptol-XE Discontinue Timoptol after proper dosing on 1 day & start treatment w/ same conc of Timoptol-XE the following day. Transfer from another topical ophth β-blocker Discontinue β-blocker after proper dosing on 1 day & start treatment w/ 1 drop the following day. Transfer from single antiglaucoma agent other than topical ophth β-blocker Continue agent & add 1 drop to each affected eye once daily. Discontinue previously used antiglaucoma agent the following day & continue Timoptol-XE.
Contraindications
Hypersensitivity. Reactive airway disease, bronchial asthma or anamnestic bronchial asthma, severe COPD; sinus bradycardia; sick sinus syndrome, SA block, 2nd & 3rd degree AV block; overt cardiac failure; cardiogenic shock.
Special Precautions
History of atopy or severe anaphylactic reaction. Adequately control cardiac failure prior to therapy. History of CV disease, cardiac failure, 1st-degree heart block, mild or moderate COPD; severe peripheral circulatory disturbance/disorders eg, severe forms of Raynaud's disease or Raynaud's syndrome; angle-closure glaucoma; choroidal detachment; corneal diseases. Patients subject to spontaneous hypoglycemia or diabetic patients receiving insulin or oral hypoglycemic agents. May mask signs & symptoms of acute hypoglycemia, hyperthyroidism. Avoid abrupt w/drawal. Remove contact lens prior to administration & reinsert after 30 min. Not recommended w/ another topical β-blocker. Concomitant use w/ anesth. Pregnancy & lactation. Childn.
Adverse Reactions
Transient blurred vision, burning & stinging, conjunctival redness, discharge, foreign body sensation, itching.
Drug Interactions
Mydriasis w/ epinephrine. Additive effects w/ Ca-channel blocker, catecholamine-depleting drugs eg, reserpine, antiarrhythmics eg, amiodarone, digitalis glycosides, parasympathomimetics, guanethidine or another β-adrenergic blocker. Exacerbated rebound HTN w/ oral β-adrenergic blocker. Potentiated systemic β-blockade w/ CYP2D6 inhibitors eg, quinidine, SSRIs.
