Oxcarbazepine
Indications/Uses
Epilepsy: Partial or generalized tonic-clonic seizures.
Dosage/Direction for Use
Adult & elderly 600-2,400 mg daily in 2 divided doses. Childn 8-10 mg/kg daily in 2 divided doses.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to oxcarbazepine or eslicarbazepine.
Special Precautions
Hypersensitivity. Risk of anaphylaxis & angioedema. Discontinue use if serious dermatological reactions, seizure aggravation or significant bone marrow depression develops. Hyponatraemia; preexisting renal conditions associated w/ low Na or patients on Na-lowering co-medication; cardiac insufficiency & secondary heart failure; hypothyroidism. Suicidal ideation or behaviour. Initial testing for patients +ve on HLA-B* 1502 allele (eg, Chinese ancestry) & HLA-A* 3101. Monitor serum Na in case of fluid retention; thyroid function especially in childn ≤2 yr. Avoid abrupt discontinuation. Women taking hormonal contraceptives. Concomitant use w/ alcohol. May affect ability to drive or use machines. Renal impairment (CrCl <30 mL/min). Hepatitis & severe hepatic impairment. Women of childbearing potential should use highly effective contraception. Pregnancy. Not to be used during lactation.
Adverse Reactions
Somnolence, headache, dizziness; diplopia; vomiting, nausea; fatigue. Increased wt; hyponatraemia, agitation, affect lability, confusional state, depression, apathy; ataxia, tremor, nystagmus, amnesia, attention & visual disturbance, blurred vision; vertigo; diarrhoea, abdominal pain, constipation; rash, alopecia, acne; asthenia.
Drug Interactions
Decreased plasma conc of dihydropyridine Ca antagonists, OCs, antiepileptic drugs (eg, carbamazepine), immunosuppressants (eg, ciclosporin), rifampicin. Increased plasma levels of phenytoin, phenobarb. Decreased mean AUC values of ethinylestradiol, levonorgestrel. Lowered AUC values of felodipine. Minor plasma level change w/ viloxazine.
