Atorvastatin Ca
Adjunct to diet for reduction of elevated total cholesterol, LDL-cholesterol, apolipoprotein B & triglycerides in adult & childn ≥10 yr w/ primary hypercholesterolaemia, heterozygous familial hypercholesterolaemia or combined (mixed) hyperlipidaemia; homozygous familial hypercholesterolaemia. To reduce risk of CV events in diabetic patients w/ at least 1 additional risk factor, w/o clinically evident CHD irrespective of whether cholesterol is raised.
Dosage/Direction for Use
Individualized dosage. Heterozygous familial & nonfamilial hypercholesterolemia & mixed dyslipidemia (Fredrickson types IIa & IIb) Initially 10 or 20 mg once daily. Patients who require a large reduction in LDL-C Initially 40 mg once daily. Homozygous familial hypercholesterolemia 10-80 mg daily. Childn 10-17 yr Heterozygous familial hypercholesterolemia Initially 10 mg/day. Max: 20 mg/day. Adjust at intervals of ≥4 wk.
May be taken with or without food: Avoid excessive consumption (>1 L/day) of grapefruit juice.
Hypersensitivity. Active liver disease or unexplained persistent elevations of serum transaminases. Women of childbearing potential. Pregnancy & lactation.
Special Precautions
Control hypercholesterolemia w/ appropriate diet, exercise & wt reduction in obese patients & other underlying medical problems before instituting therapy.
Adverse Reactions
Insomnia; headache; abdominal pain, dyspepsia, nausea, flatulence, constipation, diarrhoea; myalgia; asthenia.
Drug Interactions
Increased risk of myopathy w/ cyclosporin, fibric acid derivatives, niacin, erythromycin, azole antifungals. Decreased plasma conc w/ antacid, colestipol. Increased plasma conc of Digoxin. Increased plasma conc w/ Erythromycin. Increased AUC w/ norethindrone & ethinyl estradiol.
