Cefoperazone 500 mg, sulbactam 500 mg
Indications/Uses
Upper & lower resp tract infections, UTI, peritonitis, cholecystitis, cholangitis & other intra-abdominal infections, bacterial sinusitis, meningitis, skin & soft tissue infections, bone & joint infections, pelvic inflammatory disease, endometritis, gonorrhea & other genital tract infections.
Dosage/Direction for Use
IM/IV Adult 2-4 g IM/IV daily (1-2 g/day each of cefoperazone & sulbactam), given in equally divided doses 12 hrly. Severe or refractory infections May be increased to 8 g IV (4 g/day each of cefoperazone & sulbactam), given in equally divided doses 12 hrly. Max: 4 g sulbactam (8 g of the combination). Childn 40-80 mg/kg/day (20-40 mg/kg/day each of cefoperazone & sulbactam) 6-12 hrly. Serious or refractory infections May be increased up to 160 mg/kg/day of the combination (80 mg/kg/day each of cefoperazone & sulbactam) 6-12 hrly.
Contraindications
Known allergy to penicillins or cephalosporins.
Special Precautions
Hypersensitivity. Pseudomembranous colitis may occur; may permit Clostridia overgrowth; discontinue use in mild cases of colitis or manage w/ fluid, electrolyte, & protein supplementation in moderate to severe cases. Disulfiram-like reaction when alcohol is ingested w/in 72 hr of cefoperazone administration may occur; do not to take w/ alcohol.
Adverse Reactions
Hypersensitivity; reversible neutropenia, transient eosinophilia; diarrhea, pseudomembranous colitis symptoms; transient elevations in BUN & serum creatinine conc; rash, skin reactions, fever, transient pain, phlebitis.
Drug Interactions
Increased risk of nephrotoxicity w/ aminoglycosides. Marked hypoprothrombinemia &/or prolonged bleeding time w/ heparin/warfarin.
