Indications/Uses
Erosive reflux oesophagitis; long-term management of healed oesophagitis to prevent relapse; symptomatic treatment of GERD. Eradication of H. pylori & healing of H. pylori-associated duodenal ulcer; prevention of peptic ulcer relapse in patients w/ H. pylori-associated ulcers. NSAID-induced gastric ulcers; prevention of NSAID-induced gastric & duodenal ulcers. Zollinger- Ellison syndrome. Prolonged treatment after IV-induced prevention of peptic ulcer bleeding.
Dosage/Direction for Use
Adult & adolescent ≥12 yr Erosive reflux oesophagitis 40 mg once daily for 4 wk. May give additional 4 wk treatment for patients in whom oesophagitis has not healed or who have persistent symptoms. Long-term management of healed oesophagitis to prevent relapse 20 mg once daily. Symptomatic treatment of GERD w/o oesophagitis 20 mg once daily. Adult Eradication of H. pylori & healing of H. pylori-associated duodenal ulcer, & prevention of peptic ulcer relapse in patient w/ H. pylori-associated ulcer Combination therapy: Esomeprazole 20 mg bd w/ amoxicillin 1 g bd & clarithromycin 500 mg bd, for 7 days. NSAID-induced gastric ulcer 20 mg once daily. Treatment duration: 4-8 wk. Zollinger-Ellison syndrome Initially, 40 mg bd. Individually adjust dose & continue treatment as long clinically indicated. Prolonged treatment after IV-induced prevention of peptic ulcer rebleeding 40 mg once daily for 4 wk after IV-induced prevention of peptic ulcer rebleeding. Severe hepatic impairment Max: 20 mg.
Administration
Should be taken on an empty stomach: Take at least 1 hr before meals. Swallow whole, do not chew/crush.
Contraindications
Special Precautions
Exclude malignancy of alarm symptom (eg, unintentional wt loss, recurrent vomiting, dysphagia, haematemesis or melaena) for possible alleviation of symptoms & delayed diagnosis. Patient on long-term treatment (particularly those treated for >1 yr). Contains sucrose; not for patient w/ rare hereditary problems of fructose intolerance, glucose-galactose malabsorption or sucrase-isomaltase insufficiency. Slightly increased risk of GI infections eg, Salmonella, Campylobacter. Co-administration w/ atazanavir is not recommended. Severe renal insufficiency. Pregnancy. Do not use during lactation.
Adverse Reactions
Headache, abdominal pain, diarrhea, flatulence, nausea/vomiting, constipation.
Drug Interactions
May decrease ketoconazole & itraconazole absorption. May increase plasma conc of diazepam, citalopram, imipramine, clomipramine, phenytoin. Monitor plasma phenytoin conc upon esomeprazole introduction or w/drawal.
