Heparin Na
Prophylaxis & treatment of venous thrombosis & pulmonary embolism. Treatment of MI & arterial embolism. Prevention of clotting in arterial & heart surgery & prevention of cerebral thrombosis. As an anticoagulant in blood transfusions, extracorporeal circulation, dialysis & for lab purposes.
Dosage/Direction for Use
IV 5,000-10,000 IU 4 hrly by bolus inj or continuous infusion. SC 10,000 IU 8 hrly after an initial IV bolus inj of 5,000 IU. Low-dose heparin prophylaxis 5,000 IU in 0.2 mL SC, 2-6 hr pre-op & 8-12 hrly post-op for 10-14 days or until patient is mobile, whichever is longer. MI 5,000 IU SC 12 hrly beginning during 12 hr following the 1st sign of MI. Open heart surgery Operations <2 hr: 120 IU/kg/hr. Operations of longer duration: Give 1 1/2 times the dose. 2,000 IU is needed for each 450 mL of blood used. Perioperative prophylaxis 10-14 days to 6 wk. Elderly Reduce dose in women.
Hypersensitivity. Suitable blood coagulation tests eg, the whole-blood clotting time, partial thromboplastin time, cannot be performed at the required intervals. Uncontrolled active bleeding state. Use heparin w/o preservative in premature infants.
Special Precautions
Discontinue promptly if coagulation test is prolonged or haemorrhage occurs. Patients who are about to undergo surgery. Impaired renal or hepatic function. Continue heparin in slightly decreasing doses for another 4-5 days until the oral drug has attained full prothrombin depressing activity, if oral anticoagulants are started. Give a trial dose of 1,000 units before giving a therapeutic dose. Pregnancy (risk of osteoporosis)
Adverse Reactions
Transient alopecia, diarrhoea, thrombocytopenia, osteoporosis w/ spontaneous fractures.
Drug Interactions
Drugs that interfere w/ platelet-aggregation reactions & may induce bleeding eg, acetylsalicylic acid, dextran, phenylbutazone, ibuprofen, indomethacin, dipyridamole & hydroxychloroquine. Digitalis, tetracycline, nicotine, or antihistamines may partially counteract the anticoagulant action of heparin Na.
