Losartan K 50 mg, hydrochlorothiazide 12.5 mg
Indications/Uses
HTN not adequately controlled on individual components.
Dosage/Direction for Use
50 mg/12.5 mg once daily, may be increased to 100 mg/25 mg once daily after several wk if necessary. Elderly >75 yr Moderate to severe renal impairment, IM vol depletion 25 mg/6.25 once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to losartan, hydrochlorothiazide or other sulfonamide derivatives. Anuria. Pregnancy (2nd & 3rd trimester) & lactation.
Special Precautions
Renal impairment. Do not use in hepatic impairment. Pregnancy (1st trimester).
Adverse Reactions
Palpitations, orthostatic effects, angina pectoris, arrhythmias (eg, atrial fibrillation, sinus bradycardia, tachycardia, ventricular tachycardia/fibrillation), CVA, hypotension, MI, 2nd-degree AV-block; orthostatic hypotension; dizziness, syncope, anxiety, ataxia, confusion, depression, dream abnormality, hyperesthesia, insomnia, decreased libido, memory impairment, migraine, nervousness, panic disorder, paresthesia, peripheral neuropathy, sleep disorder, somnolence, tremor, vertigo; restlessness; sinusitis, cough, nasal congestion, angioedema, pharyngeal discomfort, rhinitis, blurred vision, burning/stinging in the eyes, conjunctivitis, decreased visual activity, taste perversion, tinnitus; transient blurred vision, xanthopsia; abdominal pain, anorexia, constipation, dental pain, dry mouth, dyspepsia, flatulence, gastritis, vomiting; pancreatitis, sialadenitis, cramping, gastric irritation; impotence, nocturia, urinary frequency, UTI; glucosuria, renal failure, renal dysfunction, interstitial nephritis; anemia, aplastic anemia, agranulocytosis, leukopenia, hemolytic anemia, thrombocytopenia; jaundice (intrahepatic cholestatic jaundice); edema, gout, hyperglycemia, hyperuricemia; upper resp tract infection, dyspepsia, epistaxis, resp distress (eg, pneumonitis, pulmonary edema).
Drug Interactions
Losartan: Elevated plasma levels w/ fluconazole & lithium. Reduced plasma levels w/ rifamycins (eg, rifampin). May increase serum K w/ K-sparing diuretics (eg, spironolactone), K supplements, salt substitutes containing K. Hydrochlorothiazide: May increase risk of orthostatic hypotension w/ alcohol, barbiturates, narcotics. Antidiabetic agents. May potentiate actions of other antihypertensives. Absorption may be impaired by cholestyramine, colestipol resins. Increased risk of electrolyte depletion (eg, hypokalemia) w/ ACTH, corticosteroids. May decrease response to pressor amines (eg, norepinephrine). Increased responsiveness to nondepolarizing muscle relaxants (eg, tubocurarine). May elevate lithium plasma levels & increase the risk of lithium toxicity. Reduced antihypertensive, diuretic & natriuretic effects w/ NSAIDs.
