Quinapril HCl
Dosage/Direction for Use
HTN Patient not on diuretics Initially 10 or 20 mg once daily. Maintenance: May be titrated to 20-40 mg/day as single or in 2 divided doses, depending upon response. Patient w/ concomitant use of diuretic Initially 5 mg/day, subsequently titrated to attain optimal response. CHF Initially 5 mg once or bid. If tolerated, may be titrated to 10-40 mg/day in 2 divided doses. Renal impairment CrCl >60 mL/min 10 mg. 30-60 mL/min 5 mg. 10-30 mL/min 2.5 mg. Elderly Initially, 10 mg once daily.
Administration
Should be taken on an empty stomach.
Contraindications
Hypersensitivity. Increased risk of angioedema when combined w/ sacubitril/valsartan. Do not co-administer w/ aliskiren in patients w/ diabetes, moderate to severe kidney insufficiency, hyperkalemia, CHF patients who are hypotensive. Do not co-administer w/ angiotensin receptor blocker or other ACE inhibitor. Women w/ childbearing potential. Pregnancy.
Special Precautions
Head & neck angioedema. Intestinal angioedema. Anaphylactoid reactions. Dual blockage of the renin-angiotensin system. Hypotension. Impaired renal & hepatic function. Hyperkalemia. Hyponatremia & syndrome of inappropriate antidiuretic hormone. Fetal morbidity/mortality. Hyperglycemia & diabetes. Cough. Surgery & anesth. May impair to drive or operate machinery. Lactation.
Adverse Reactions
Headache, dizziness, cough, fatigue, rhinitis, nausea &/or vomiting, myalgia.
Drug Interactions
May reduce absorption of tetracycline & other drugs that interact w/ Mg. May increase risk of lithium toxicity. May result in deterioration of renal function including selective cyclooxygenase-2 inhibitors. Increased in serum K w/ K-sparing diuretics, K supplements, K-containing salt substitutes or other drugs known to raise serum K. Hyperkalemia w/ trimethoprim-containing products. Dual blockage of the RAS w/ angiotensin receptor blockers, ACE inhibitors, or aliskiren.
