Palonosetron HCl

Cap: Prevention of nausea & vomiting associated w/ moderately emetogenic cancer chemotherapy in adults. 75 mcg/1.5 mL IV: Prevention of post-op nausea & vomiting for up to 72 hr following surgery. 250 mcg/5 mL IV: Adult: Prevention of acute & delayed nausea & vomiting associated w/ initial & repeat courses of moderately & highly emetogenic cancer chemotherapy. Childn 1 mth to <17 yr: Prevention of acute nausea & vomiting associated w/ initial & repeat courses of emetogenic cancer chemotherapy, including highly emetogenic cancer chemotherapy.

Cap Adult 500 mcg PO approx 1 hr before start of chemotherapy. 75 mcg/1.5 mL IV Adult 75 mcg as single dose over 10 sec immediately before induction of anesth. 250 mcg/5 mL IV Adult 250 mcg infuse over 30 sec beginning approx 30 min before chemotherapy. Childn 1 mth to <17 yr 20 mcg/kg (max: 1.5 mg) infuse over 15 min beginning approx 30 min before chemotherapy.

May be taken with or without food.

Hypersensitivity.

Monitor for serotonin syndrome. Pregnancy & lactation. Elderly. Cap: Hypersensitivity to peanut or soya. Monitor patients w/ history of constipation or signs of subacute intestinal obstruction. Patients w/, or are likely to develop prolongation of the QT interval especially those w/ electrolyte abnormalities, CHF, bradyarrhythmias, conduction disturbances, & taking antiarrhythmic agents or medicinal products that may lead to QT prolongation or electrolyte abnormalities. Correct hypokalemia & hypomagnesemia prior to administration. Do not use in prevention or treatment of nausea & vomiting in the days following chemotherapy if not associated w/ another chemotherapy administration. Fructose intolerance. May impair ability to drive or operate machinery. IV: Monitor for emergence of serotonin syndrome, especially w/ concomitant use w/ other serotonergic drugs. Discontinue treatment if serotonin syndrome occurs. Labour & delivery. 75 mcg/mL IV: Childn for post-op nausea & vomiting. 250 mcg/mL IV: Neonates <1 mth.

Headache. IV: ECG QT prolongation, bradycardia, constipation, diarrhea, dizziness, fatigue, abdominal pain, insomnia.

Serotonin syndrome w/ concomitant use of 5-HT₃ receptor antagonists & other serotonergics including SSRIs & SNRIs.

Antiemetics

A04AA05 - palonosetron ; Belongs to the class of serotonin (5HT3) antagonists. Used for the prevention of nausea and vomiting.
