Amphotericin B
Potentially life-threatening fungal infections eg, cryptococcosis (torulosis); North American blastomycosis; disseminated forms of moniliasis (candidiasis), coccidioidomycosis & histoplasmosis; mucormycosis (phycomycosis); sporotrichosis (Sporothrix schenckii); aspergillosis (Aspergillus fumigatus); American mucocutaneous leishmaniasis.
Dosage/Direction for Use
IV infusion over a period of 2-4 hr. Initially 0.25 mg/kg, gradually increased to 1 mg/kg depending on individual response & tolerance. Seriously ill patient Dose may be gradually increased up to 1.5 mg/kg.
Hypersensitivity. Pregnancy & lactation. Childn.
Special Precautions
Not to be used for common non-invasive fungal infections eg, oral thrush, vag candidiasis & esophageal candidiasis in patients w/ normal neutrophil counts.
Adverse Reactions
Fever, malaise, loss of mass; hypotension, tachypnea; anorexia, nausea, vomiting, diarrhea, dyspepsia, cramping epigastric pain; normochromic, normocytic anemia; pain at the inj site w/ or w/o phlebitis or thrombophlebitis; generalized pain, including muscle & joint pains; headache; decreased renal function & renal function abnormalities including azotemia, increased serum creatinine, hypokalemia, hyposthenuria, renal tubular acidosis, & nephrocalcinosis.
Drug Interactions
May enhance potential for renal toxicity w/ other nephrotoxic drugs eg, cisplatin, pentamidine, aminoglycosides, & cyclosporine. May potentiate induced hypokalemia w/ corticosteroids & corticotropin (ACTH). Effects or toxicity of digitalis glycosides, skeletal muscle relaxants & antiarrhythmic agents may be increased by hypokalemia. May increase toxicity w/ flucytosine. Acute pulmonary reactions after leukocyte transfusions. May induce fungal resistance w/ imidazoles (eg, ketoconazole, miconazole, clotrimazole, fluconazole).
