Bambuterol HCl
Indications/Uses
Bronchial asthma, chronic bronchitis, emphysema & other lung diseases where bronchospasm is a complicating factor.
Dosage/Direction for Use
Once daily dosing preferably during bedtime. Adult, elderly, childn >12 yr Initially 10 mg, may be increased to 20 mg after 1-2 wk. Patient who have well tolerated B2-agonists Initially 20 mg. Impaired renal function (GFR ≤50 mL/min) Initially 5 mg, may be increased to 10 mg after 1-2 wk. Childn 6-12 yr Initially 10 mg, may be increased to 20 mg after 1-2 wk. 2-5 yr 5 mg (in East Asian childn) & 10 mg (normal dose).
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to bambuterol HCl or terbutaline.
Special Precautions
Thyrotoxicosis. Patients w/ underlying severe heart disease (eg, ischaemic heart disease, arrhythmia or severe heart failure). Not for the treatment of premature labour. Do not use as a tocolytic in patients w/ preexisting ischaemic heart disease or those w/ significant risk factors for ischaemic heart disease. Recommend additional blood glucose controls in diabetic patients. Hypokalemia; monitor serum K levels. Patients w/ persistent asthma should receive optimal anti-inflammatory therapy eg, inhaled corticosteroids, leukotriene receptor antagonists. Liver cirrhosis, severe hepatic & renal impairment. Pregnancy (1st trimester) & lactation.
Adverse Reactions
Tremor, headache; behavioral disturbances eg, restlessness. Palpitations; tonic muscle cramps; sleep disturbances.
Drug Interactions
Prolongs the muscle-relaxing effects of suxamethonium (succinylcholine) & other muscle relaxants. β-blockers (including eyedrops) may partly or totally inhibit effects of β-stimulants. Increased risk of cardiac arrhythmias w/ halothane anaesth. Exacerbated risk of hypokalemia w/ diuretics, methylxanthines & corticosteroids. Hypokalaemia also predisposes to digoxin toxicity.
