Terbutaline sulfate
Bronchial asthma, chronic bronchitis, emphysema & other lung diseases where bronchospasm is a complicating factor. Pre-term labor.
Dosage/Direction for Use
Tab Bronchospasm Adult 2.5 mg 3 times in a 24 hr period during the 1st 1-2 wk, then may be increased to 5 mg tid. Childn 0.075 mg/kg 3 times in 24 hr; suitable dosage: >30 kg 1-2 tab 3 times in 24 hr, 20-30 kg ½-1 tab 3 times in 24 hr, 20 kg ¼-½ tab 3 times in 24 hr. Inj Bronchospasm Adult IV inj 0.25-0.5 mg slow IV inj for 5 min. Max: 2 mg in 24 hr. IV infusion 1-2 mg 24 hrly by continuous infusion. Initial loading dose: Up to 0.10 mg over 10 min. SC 1-2 mg 24 hrly, split into at least 4 occasions. Childn IV infusion Up to 25 mcg/kg 24 hrly as continuous infusion. Initial loading dose: Up to 1.5 mcg/kg over 10 min. SC Up to 25 mcg/kg 24 hrly, split into at least 4 occasions. Pre-term labor Initially 5 mcg/min IV infusion for 20 min, increased by 2.5 mcg/min at 20-min intervals until contraction stops. >10 mcg/min should seldom be given & 20 mcg/min should not be exceeded. Continue for 1 hr at the chosen infusion rate & then decrease rate in steps of 2.5 mcg/min at 20-min intervals to the lowest dose for that produces continued suppression of contractions 12 hr. Continue w/ oral maintenance therapy 5 mg tid until the end of 36th wk of pregnancy or 0.25 mg SC inj 4 times in a 24 hr period for a few days before oral treatment.
May be taken with or without food.
Hypersensitivity. Inj: In obstet, intrauterine infection, severe preeclampsia, ablatio placentae.
Special Precautions
Thyrotoxicosis, severe CV disorders eg, ischemic heart disease, tachyarrhythmias or severe heart failure; diabetics. Acute severe asthma. Additional blood glucose controls in diabetic patients. Risk of serious hypokalemia. Monitor K levels. Pregnancy & lactation.
Adverse Reactions
Tremor, headache, nausea, tonic muscle cramps, tachycardia & palpitations. Urticaria & exanthema. Sleep & behavioral disturbances eg, agitation, hyperactivity & restlessness. Inj (on preterm labour): Hyperglycemia & lactic acidosis in diabetic mothers, hypokalemia w/ high doses. Pulmonary edema & increased tendency to bleed w/ cesarean section.
Drug Interactions
May partly or totally inhibit effect of β-receptor blocking agents (including eye drops), especially non-selective. Hypokalemia may be potentiated by concomitant treatment w/ xanthine derivatives, steroids & diuretics.
ATC Classification
R03CC03 - terbutaline ; Belongs to the class of adrenergics for systemic use, selective beta-2-adrenoreceptor agonists. Used in the treatment of obstructive airway diseases.
