Dobutamine HCl
Indications/Uses
Inotropic support in short-term treatment of adults w/ cardiac decompensation due to depressed contractility resulting either from organic heart disease or from cardiac surgical procedures.
Dosage/Direction for Use
Adult & childn Initially 0.5-1 mcg/kg/min, increased to 2.5-15 mcg/kg/min. Max: 40 mcg/kg/min for adult.
Contraindications
Hypersensitivity to sulfites. Idiopathic hypertrophic subaortic stenosis, atrial fibrillation/flutter.
Special Precautions
Do not administer at same IV line as heparin, hydrocortisone Na succinate, cefazolin or penicillin. Hypovolemia should be corrected prior to use. Extravasation may cause dermal necrosis. MI, marked mechanical obstruction eg, severe valvular stenosis. Long-term treatment (>48 hr) of patients w/ CHF. Pregnancy & lactation.
Adverse Reactions
Ectopic heartbeats, increased heart rate, chest pain, angina, palpitations, elevated BP, ventricular tachycardia or arrhythmias at high doses; atrial fibrillation or flutter; tingling sensation, headache; nausea & vomiting; mild leg cramps, paresthesia; dyspnea.
Drug Interactions
Increased peripheral resistance w/ β-adrenergic blockers. Increased toxicity w/ general anesth eg, halothane & cyclopropane.
