Role of S-Adenosyl L-Methionine is thought to be defined by the amalgamation of the three different roles as mentioned under: Role in Oxidative Stress: Oxidative Stress was shown to play a major pathogenic role in multiple disease states ranging from the hepatotoxicity of alcohol (and other xenobiotics) to the carcinogenicity of many compounds. The major natural defense mechanism against oxidative stress is reduced glutathione, which traps the excess of free radicals. Glutathione is a tripeptide, the rate-limiting amino acid being cysteine, and SAMe plays a fundamental role in the formation of cysteine.
Role in Trans-methylation and Trans-sulfuration reactions: Another basic cellular activity of SAMe is its role as a methyl donor and enzyme activator in the transmethyiation and transsulfuration reactions key to membrane structure and function. For example, SAMe is essential for the transport processes and signal transmission across membranes. One of the important consequences of the failure of these functions is insufficiency of bile formation, a key aspect of many diseases of the liver, resulting in a pathologic state called cholestasis. SAMe opposes successfully many of the cholestatic states, as reviewed elsewhere. SAMe improves both the pruritus and the biochemical indexes of cholestasis, such as serum bilirubin, alkaline phosphatase, and y-glutamyltransferase.
Role in opposing Fibrosis: The leading cause of morbidity and mortality in all major liver diseases is an inappropriately excessive healing process with uncontrolled scarring or fibrosis culminating in cirrhosis. Indeed, fibrosis can be viewed as an initially beneficial scarring process that has escaped control and results ultimately in cirrhosis. SAMe was shown to be therapeutically useful in alleviating this process experimentally and for improving the outcome clinically. The most common liver disease for which SAMe has been shown to be useful therapeutically is alcoholic liver injury, which encompasses all the pathologic manifestations discussed above, namely a deficiency in the activation of methionine to SAMe, in the pathogenic role of oxidative stress and glutathione deficiency, in complications of cholestasis, and in the devastating consequences of excessive liver fibrosis (leading to cirrhosis).
CIROZE 400 is indicated for management of liver inflammation caused by different conditions including alcoholic liver cirrhosis.
The recommended dosage of CIROZE 400 is 400-1200 mg per day orally in divided doses for minimum 6 months to enhance liver function. A qualified healthcare provider may suggest otherwise depending on the condition of the patient.
CIROZE 400 should be taken on an empty stomach. (Swallow whole, do not chew/crush.)

S-Adenosyl-L-Methionine can cross the placental barrier and be delivered through breast milk; however it is unclear what danger this exposure to S-Adenosyl-L-Methionine may pose to developing child. Until further research has been done, S-Adenosyl-L-Methionine should be avoided or if required, it should be under the advice of a healthcare professional.
Taking S-Adenosyl-L-Methionine at the same time as these drugs may increase the risk of serotonin syndrome (a potentially dangerous condition caused by having too much serotonin in body): Dextromethorphan; Meperidine; Pentazocine; Tramadol.
Antidepressant medications: S-Adenosyl-L-Methionine may interact with antidepressant medications, increasing the potential for side effects including headache, irregular or accelerated heart rate, anxiety, and restlessness. Levodopa (L-dopa)-S-Adenosyl-L-Methionine may reduce the effectiveness of this medication for Parkinson's disease. A medication for diabetes - S-Adenosyl-L-Methionine may reduce levels of blood sugar and may strengthen the effect of diabetes medications, which increases the risk of hypoglycemia (low blood sugar).
