Indications/Uses
Renal tubular acidosis (RTA) w/ Ca stones, hypocitraturic Ca oxalate nephrolithiasis of any etiology & uric acid lithiasis w/ or w/o Ca stones.
Dosage/Direction for Use
Severe hypocitraturia (urinary citrate <150 mg/day) 60 mEq/day (20 mEq tid or 15 mEq qid. Mild-moderate hypocitraturia (>150 mg/day) 30 mEq/day (10 mEq tid).
Administration
Should be taken with food.
Contraindications
Hyperkalemia; chronic renal failure, uncontrolled DM, acute dehydration, strenuous physical exercise in unconditioned individuals, adrenal insufficiency, extensive tissue breakdown. K-sparing agent (eg, triamterene, spironolactone or amiloride). Patients suffering from delayed gastric emptying, esophageal compression, intestinal obstruction or stricture or those taking anticholinergics; PUD. Renal insufficiency (GFR <0.7 mL/kg/min).
Special Precautions
Patients w/ impaired mechanisms for excreting K, severe myocardial damage or heart failure. Concomitant administration w/ K-sparing diuretic (eg, triamterene, spironolactone or amiloride). Discontinue if severe vomiting, abdominal pain or GI bleeding occurs. Pregnancy & lactation. Childn.
Adverse Reactions
Minor GI complaints eg, abdominal discomfort, vomiting, diarrhea, loose bowel movements or nausea. Intact matrices in feces.
Drug Interactions
May produce severe hyperkalemia w/ K-sparing diuretic (eg, triamterene, spironolactone or amiloride).
