Contents

Dosage/Direction for Use
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to valsartan, hydrochlorothiazide or sulfonamides. Anuria. Concomitant use w/ aliskiren in type 2 diabetes. Pregnancy.
Special Precautions
Risk of hypotension in Na- &/or vol-depleted patients. Renal artery stenosis, renal & liver disease, biliary cirrhosis & cholestasis or SLE. Patients w/ history of angioedema or are experiencing angioedema; discontinue immediately & do not re-administer. Heart failure, severe chronic heart failure or other conditions w/ simulation of renin-angiotensin-aldosterone-system; renal impairment may occur. Hypokalemia (may favor the onset of digitalis-induced cardiac arrhythmias), hyponatremia, hypercalcemia, symptomatic hyperuricemia. Disturbance of serum electrolyte balance, glucose tolerance & cholesterol, triglycerides & uric acid serum levels. Acute angle-closure glaucoma. Allergy or asthma. Avoid concomitant use w/ aliskiren in severe renal impairment (GFR <30 mL/min). Concomitant use w/ ACE inhibitors or aliskiren. Increased risk of non-melanoma skin cancer (NMSC) [eg, basal cell carcinoma (BCC) & squamous cell carcinoma (SCC)] w/ increasing cumulative dose of hydrochlorothiazide. Protect skin from excessive sun exposure. Not used in women planning to get pregnant. Pregnancy & lactation.
Adverse Reactions
Hydrochlorothiazide: Hypokalemia & rise in blood lipids, hypomagnesemia, hyperuricemia, hyponatremia, urticaria & other forms of rash, decreased appetite, mild nausea & vomiting, orthostatic hypotension, erectile dysfunction.
Drug Interactions
May increase incidence of hypotension, hyperkalemia & changes in renal function w/ ACE inhibitors or aliskiren. May increase serum K levels w/ K-sparing diuretics or K supplements, salt substitutes containing K or drugs that increase serum K levels (eg, heparin). May increase lithium conc & toxicity. Hypokalemia w/ corticosteroids, ACTH, amphotericin, penicillin G, carbenoxolone, antiarrhythmics. Hyponatremia w/ antidepressants, antipsychotics, antiepileptics. May decrease antihypertensive effects w/ NSAIDs including Cox-2 inhibitors. Electrolyte imbalance w/ digitalis glycosides. Other antihypertensives; insulin & oral antidiabetics; anionic exchange resins, allopurinol, amantadine, diazoxide, cytotoxic drugs, anticholinergic agents, vit D, Ca salts, ciclosporin, methyldopa, pressor amines (eg, noradrenaline), barbiturates, narcotics & alcohol. Valsartan: May increase systemic exposure w/ inhibitors of the uptake transporter (eg, rifampin, ciclosporin) or efflux transporter (ritonavir).
