Indications/Uses
Parenteral nutrition for adults & childn >2 yr when oral or enteral nutrition is impossible, insufficient or contraindicated.
Dosage/Direction for Use
Combiflex Lipid Central IV infusion into central vein Adult & childn >10 yr 19-38 mL/kg/day. Childn 2-10 yr 12.5-25 mL/kg, increased by 10-15 mL/kg/day up to max of 40 mL/kg/day. Infusion rate: Should not exceed 2.6 mL/kg/hr for 12-24 hr. Combiflex Lipid Peri Adult & childn >10 yr 27-40 mL/kg/day. Childn 2-10 yr 14-28 mL/kg, increased by 10-15 mL/kg/day up to max of 40 mL/kg/day. Infusion rate: Should not exceed 3.7 mL/kg/hr for 12-24 hr.
Contraindications
Hypersensitivity to egg, soya or peanut protein. Severe hyperlipemia, liver insufficiency & blood coagulation disorders; inborn errors of amino acid metabolism, severe renal insufficiency w/o access to hemofiltration or dialysis, acute shock, hyperglycemia which requires insulin >6 units/hr, pathologicallly elevated serum levels of any included electrolytes. Acute pulmonary edema, hyperhydration & decompensated cardiac insufficiency & hypotonic dehydration, haemophagocytotic syndrome, unstable conditions (eg, severe post-traumatic conditions, uncompensated diabetes, acute MI, metabolic acidosis, severe sepsis & hyperosmolar coma). Infant <2 yr.
Special Precautions
Monitor ability to eliminate lipid (serum triglyceride conc should not exceed 3 mmol/L during infusion). Correct electrolyte & fluid disturbances before infusion. Patients w/ impaired lipid metabolism due to renal insufficiency, uncompensated DM, pancreatitis, impaired liver function, hypothyroidism (w/ hypertriglyceridemia) or sepsis. Monitor serum triglyceride, serum glucose, electrolytes, osmolarity, fluid balance, acid-base status & liver enzyme tests; blood cell count & coagulation during prolonged use. Control phosphate & K intake in patients w/ renal insufficiency. Patients w/ metabolic acidosis (lactatic acidosis), increased serum osmolarity or those in need of fluid resuscitation & w/ tendency towards electrolyte retention. Interrupt infusion immediately if anaphylactic reaction occur. May interfere lab measurements eg, bilirubin, lactate dehydrogenase, oxygen saturation & Hb. Contains soy bean oil & egg phospholipids; cross-allergic reaction between soybean & peanut. Additional supplements of trace elements may be required in long-term IV nutrition. Malnourished patients & those w/ hyperglycemia. Pregnancy & lactation. Combiflex Lipid Peri: Thrombophlebitis.
Adverse Reactions
Increased body temp, shivering, chills, nausea/vomiting; transient increases in liver enzymes; thrombophlebitis; lipid overload syndrome; hypersensitivity reactions, resp symptoms (eg, tachypnea) & hyper/hypotension; haemolysis, reticulocytosis, abdominal pain, headache, tiredness & priapism.
Drug Interactions
Should not be given simultaneously w/ blood or blood products in the same infusion set.
