Concise Prescribing Info
Per Combiwave SF 50 MDI Salmeterol xinafoate 25 mcg, fluticasone propionate 50 mcg. Per Combiwave SF 125 MDI Salmeterol xinafoate 25 mcg, fluticasone propionate 125 mcg. Per Combiwave SF 250 MDI Salmeterol xinafoate 25 mcg, fluticasone propionate 250 mcg
Treatment of asthma where combination therapy of bronchodilator & inhaled costicosteroid is appropriate: In patients not adequately controlled w/ inhaled corticosteroids & 'as needed' inhaled short-acting β-2 agonist; patients already adequately controlled on both inhaled corticosteroid & long-acting β-2 agonist.
Dosage/Direction for Use
Adult & adolescent >12 yr 2 inhalations bid. Childn ≥4 yr 2 inhalations of 25 mcg/50 mcg bid. Max of 100 mcg bid.
Special Precautions
Should not be used as treatment for acute asthma symptoms. May worsen conditions if use during exacerbation of asthma attack. Treatment should not be stopped abruptly. Patients w/ pulmonary TB, severe CV disorders, heart rhythm abnormalities, DM, thyrotoxicosis, uncorrected hypokalemia or patients predisposed to low levels of serum K. Discontinue use, assess patient & institute alternative therapy if increase in wheezing after dosing occur. Possible systemic effects may occur during long-term therapy. Avoid concomitant use w/ ritonavir, ketoconazole or other potent CYP3A4 inhibitors. May develop pneumonia in patients w/ severe COPD. Black African or Afro-Caribbean patients. Pregnancy & lactation.
Adverse Reactions
Headache; nasopharyngitis. Candidiasis of the mouth & throat, pneumonia, bronchitis; hypokalemia; hoarseness/dysphonia, sinusitis; contusions; traumatic fractures, arthralgia, myalgia.
Drug Interactions
Avoid use w/ non-selective & selective β-blockers; β-adrenergic-containing drugs. Fluticasone: Increased plasma conc w/ ritonavir. Increased systemic exposure w/ ketoconazole & itraconazole. Salmeterol: Increased risk of systemic side effects w/ ketoconazole.
