Dobutamine HCl
Indications/Uses
Short-term treatment of heart failure. Increase the contractility of the heart in acute heart failure, as occurs in cardiogenic shock & MI; septic shock; cardiac surgery & +ve end-expiratory pressure.
Dosage/Direction for Use
Acute heart failure 2.5-10 mcg/kg/min. Exercise in cardiac stress testing 5 mcg/kg/min infused 8 min, increased by increments of 5 mcg/kg/min, max: 20 mcg/kg/min infused for 8 min before the next incremental increase.
Contraindications
Avoid in patients w/ marked obstruction of cardiac ejection, eg, idiopathic hypertrophic subaortic stenosis.
Special Precautions
Patients w/ acute MI & cardiogenic shock complicated by severe hypotension; preexisting arrhythmias or tachycardia, Prinzmetal's angina, thromboembolic disorders or history of ischemic heart disease; hyperthyroidism, elevated thyroid hormone conc. Correct hypovolemia before treatment. Falsely decreased creatinine values in enzymatic test. Anaphylactic symptoms & life threatening or less severe asthmatic episodes due Na metabisulfite content. Elderly who may have preexisting coronary or cerebrovascular disease.
Adverse Reactions
Dose-related increases in heart rate & BP, ectopic beats, angina or chest pain, palpitations. Anxiety, fear, restlessness, insomnia, confusion, irritability & psychotic states, dyspnea, hyperglycemia, palpitations, tachycardia (sometimes w/ anginal pain), tremors, sweating, hypersalivation, weakness, dizziness, headache & coldness of extremities. Anorexia, nausea, vomiting.
Drug Interactions
Cyclopropane, halothane & other volatile anesth.
