Nalbuphine HCl
Relief of moderate to severe pain, including that associated w/ MI. As adjunct to anesth.
Dosage/Direction for Use
Pain relief 10-20 mg every 3-6 hr. MI 10-30 mg by slow IV inj; a 2nd dose of 20 mg may be given after 30 min if necessary. Childn Initially 300 mcg/kg, repeated once or twice as necessary. Premed 100-200 mcg/kg. Adjunct to anesth 0.3-1 mg/kg IV over 10-15 min at induction. Maintenance: 250-500 mcg/kg given at ½-hrly intervals.
Acute resp depression & obstructive airway disease; acute alcoholism, convulsive disorders, head injuries & conditions w/ increased intracranial pressure; comatose patients. Concomitant use of MAOIs or w/in 14 days of discontinuing such treatment.
Special Precautions
May precipitate w/drawal symptoms in patients physically dependent on opioids. Hypothyroidism, adrenocortical insufficiency, asthma or decreased resp reserve, prostatic hyperplasia, hypotension, shock, inflammatory or obstructive bowel disorders or myasthenia gravis. Avoid abrupt w/drawal. May precipitate w/drawal symptoms in physically dependent patients who have recently used pure agonists eg, morphine. Development of psychological & physical dependence (repeated use). May affect ability to drive or operate machinery. Renal or hepatic impairment. Pregnancy. Infants especially neonates. Elderly or debilitated patients.
Adverse Reactions
Drug Interactions
Risk of serious & sometimes fatal reactions w/ MAOIs (including moclobemide); life-threatening reactions w/ selegiline. Enhanced depressant effects w/ other CNS depressants eg, alcohol, anesth, anxiolytics, hypnotics, TCAs & antipsychotics. May counteract haemodynamic benefits w/ cyclizine. Inhibited metabolism w/ cimetidine. GI effects may delay absorption w/ mexiletine; counteractive w/ cisapride, metoclopramide or domperidone. May reduce ciprofloxacin serum conc.
