Metformin HCl
Type 2 DM in obese & juvenile diabetics in whom diet alone has failed as monotherapy. Adjunct to diet & exercise to improve glycemic control. Fornidd Type 2 DM in adults, particularly in overwt patients, when dietary management & exercise alone does not result in adequate glycemic control as monotherapy or in combination w/ other oral antidiabetic agents, or w/ insulin. Reduction of diabetic complications in overwt type 2 diabetic patients treated as 1st-line therapy after diet failure.
Dosage/Direction for Use
Fornidd tab Adult Monotherapy & in combination w/ other oral antidiabetic agents 1 tab bid or tid. Max: 3 g daily. Combination w/ insulin 1 tab bid-tid while insulin dosage is adjusted on the basis of blood glucose measurements. GFR should be assessed before initiation of treatment in patients w/ renal impairment. Renal impairment: GFR 60-89 mL/min Max: 3,000 mg, 45-59 mL/min Max: 2,000 mg, 30-44 mL/min Max: 1,000 mg. All doses to be divided in 2-3 daily doses. Fornidd XR tab Individualized dose. Start w/ 500 mg once daily, increase dose in increments of 500 mg wkly, up to a max of 2,000 mg once daily. If glycemic control is not achieved, consider trial of 1,000 mg bid. Adjust dose according to blood glucose levels during the 1st few mth.
Should be taken with food: XR tab: Swallow whole, do not chew.
Hypersensitivity. Renal or hepatic failure, alcoholism, diabetic ketoacidosis or pre-coma, cardiac failure. Pregnancy & lactation. Fornidd Dehydration, severe infection, shock, intravascular administration of iodinated contrast agents, resp failure, recent MI, acute alcoholic intoxication. Acute metabolic acidosis. Severe renal failure (GFR <30 mL/min). Childn. Fornidd XR NIDDM complicated by severe ketosis & acidosis, diabetic coma. Patients undergoing surgery, after severe trauma or during infections, COPD, coronary heart disease, peripheral vascular disease, hypoglycemia.
Special Precautions
Lactic acidosis. Elderly. Fornidd Energy-restricted diet in overwt patients. Concomittant use w/ insulin or sulfonylureas. Regular monitoring of TSH in patients w/ hypothyroidism. Monitor vit B12 levels. Fornidd XR Childn.
Adverse Reactions
GI disturbances eg, nausea, vomiting, diarrhea, constipation, gastric pain; metallic taste. Fornidd Abdominal pain, loss of appetite. Fornidd XR Rash, pruritus, urticaria, erythema & flushing; headache & dizziness. Impaired GI absorption of vit B12 & folic acid.
Drug Interactions
Phenprocoumon. Alcohol. Fornidd Iodinated contrast agents. May increase the risk of lactic acidosis. NSAIDs including selective COX-II inhibitors, ACE inhibitors, angiotensin II receptor blocker & diuretics especially loop diuretics. May decrease anticoagulant effects of phenprocoumon. May reduce hypoglycemic effects of levothyroxine. Fornidd XR Hyperglycemics (eg, thiazides & corticosteroids), furosemide, nifedipine, cationic drugs (eg, amiloride, digoxin, morphine, procainamide, quinidine, quinine, ranitidine, triamterene, trimethoprim, cimetidine & vancomycin). Reduced absorption by acarbose & guar gum.
