Glimepiride
Indications/Uses
NIDDM (type II diabetes), where hyperglycemia cannot be controlled adequately by diet & exercise alone. May be used in combination w/ insulin & metformin.
Dosage/Direction for Use
Initially 1 mg/day. Maintenance dose: 1-4 mg/day. Max: 6 mg/day. Combination therapy w/ insulin Recommended dose: 8 mg once daily. After starting w/ low-dose insulin, upward adjustments can be done approx wkly.
Administration
Should be taken with food: Take w/ 1st main meal.
Contraindications
Hypersensitivity to glimepiride, other sulfonylureas or sulfonamides. Insulin-dependent diabetes; diabetic coma; ketoacidosis; severe renal or hepatic function disorders. Pregnancy & lactation.
Special Precautions
Regular monitoring of glucose levels in blood & urine. Determination of the proportion of HbA1c. Regular hepatic & hematological monitoring. Symptoms of hypoglycemia. Temporary switch to insulin in stress situations eg, accidents, acute operation, infections w/ fever.
Adverse Reactions
Immune system, blood & lymphatic system, metabolism & nutrition, eye, GI, hepatobiliary, skin & SC tissue disorders.
Drug Interactions
Potentiated hypoglycemic action w/ NSAIDs, highly protein-bound drugs eg, salicylates, sulfonamides, chloramphenicol, coumarins, probenecid, MAOIs, β-blockers. Hyperglycemic effect w/ thiazides & other diuretics, corticosteroids, phenothiazines, thyroid products, estrogens, OCs, phenytoin, nicotinic acid, sympathomimetics & INH. Potential interaction w/ oral miconazole, drugs metabolized by cytochrome P450 2C9 (phenytoin, diclofenac, ibuprofen, naproxen & mefenamic acid).
