Atracurium besilate
As adjunct to general anesth to facilitate endotracheal intubation & to provide skeletal muscle relaxation during surgery or mechanical ventilation.
Dosage/Direction for Use
Adult & childn ≥2 yr Intubation Initially 0.4-0.5 mg/kg as IV bolus inj. Reduce initial dose to 0.3-0.4 mg/kg & give slowly or in divided doses over 1 min in patients w/ significant CV disease/history of severe anaphylactoid reactions or asthma. Maintenance 0.08-0.10 mg/kg as intermittent IV inj. Infant 1 mth-2 yr Initially 0.3-0.4 mg/kg.
Long-term use for maintenance intubation & muscle paralysis in tetanus, chest trauma.
Special Precautions
Clinically significant CV or resp disease; history of severe anaphylactoid reactions or asthma; myasthenia gravis, Eaton-Lambert syndrome or other neuromuscular diseases, severe electrolyte disorders or carcinomatosis; bronchial asthma. Consider possibility of life-threatening anaphylactic reactions. Use only w/ adequate anesth. Do not administer IM. Avoid long-term use in ICU patients. Monitor neuromuscular function. Obstet. Pregnancy & lactation. Infant <1 mth.
Adverse Reactions
Bronchospasm; generalized flushing, hypotension, HTN, tachycardia, bradycardia; localized skin reactions.
Drug Interactions
Isoflurane, enflurane, halothane; aminoglycosides, polymyxins, spectinomycin, tetracyclines, lincomycin, clindamycin, vancomycin; lignocaine, procainamide, quinidine; propranolol, oxprenolol; verapamil; frusemide, thiazides, acetazolamide, mannitol; trimetaphan, hexamethonium; Mg sulfate, ketamine, lithium salts, quinine, chloroquine, d-penicillamine; chlorpromazine, steroids; phenytoin, carbamazepine; other nondepolarizing neuromuscular blocking agents eg, suxamethonium.
