Enalapril maleate
Indications/Uses
Treatment of HTN & heart failure. Prophylaxis in patients w/ asymptomatic left ventricular dysfunction to delay the onset of symptomatic heart failure & to those w/ left ventricular dysfunction to reduce the incidence of coronary ischemic events including MI.
Dosage/Direction for Use
HTN Initially, 5 mg daily at bedtime. Renal impairment, on diuretic therapy, elderly Initially 2.5 mg daily. Maintenance: 10-20 mg once daily, up to 40 mg daily in severe HTN. Heart failure or asymptomatic left ventricular dysfunction Initially, 2.5 mg daily. Usual maintenance dose: 20 mg daily single dose or in 2 divided doses.
Administration
May be taken with or without food.
Contraindications
Aortic stenosis or outflow tract obstruction. Renovascular disease. Porphyria. Pregnancy.
Special Precautions
Elderly or patients w/ peripheral vascular disease or generalized atherosclerosis. Monitor renal function before & during therapy. Regular WBC counts in patients w/ collagen vascular disorders or receiving immunosuppressive therapy. History of idiopathic or hereditary angioedema.
Adverse Reactions
Hypotension, dizziness, fatigue, headache, GI disturbances, taste disturbances, persistent dry cough, skin rash, angioedema, hypersensitivity reactions, hyperkalemia, hyponatremia & blood disorders.
Drug Interactions
Excessive hypotension w/ concurrent use of diuretics, other antihypertensives or alcohol. Additive hyperkalemic effect w/ K-sparing diuretics, K supplements, ciclosporin or indomethacin. NSAIDs.
