Spironolactone
Indications/Uses
Heart failure to treat refractory edema & in lower doses as an adjunct to standard therapy. Refractory edema associated w/ liver cirrhosis or nephrotic syndrome & in ascites associated w/ malignancy.
Dosage/Direction for Use
Adult 25-200 mg/day. Childn Initially 3 mg/kg daily in divided doses.
Administration
Should be taken with food: Take w/ food or milk to avoid GI discomfort. Do not take at bedtime.
Contraindications
Hypersensitivity. Hyponatraemia, hyperkalemia; severe kidney disease or are not passing urine; Addison's disease. Pregnancy & lactation.
Special Precautions
Severely ill, diabetics or patients w/ kidney problems. Monitor BP regularly; K & Na intake. Maintain adequate fluid intake. K & Na diet. Elderly.
Adverse Reactions
Confusion, cough, hoarseness, dry mouth, increased thirst, enlarged breasts in males, fast or irregular heartbeat, palpitations, chest pain, fever, chills, lower back or side pain, nervousness; numbness or tingling in hands, feet or lips; pain or difficulty passing urine, shortness of breath; difficult breathing, skin rash, itching, unusual bleeding, unusual tiredness or weakness, weakness or heaviness of legs.
Drug Interactions
Ibuprofen, corticosteroids, cyclosporine, digoxin, heparin, lithium, medicines for high BP, MAOIs, K salts, water pills.
