Phenytoin Na
Antiepileptic used to control partial & generalized tonic-clonic seizures. Used as part of the emergency treatment of status epilepticus & prophylactic control of seizures associated w/ neurosurgery or severe traumatic injury to the head. Treatment of trigeminal neuralgia; cardiac arrhythmias.
Dosage/Direction for Use
Initially 3-4 mg/kg daily or 150-300 mg daily in single or divided doses, increased to 600 mg if necessary. Maintenance dose: 200-500 mg daily. Childn Initially 5 mg/kg daily in 2 or 3 divided doses up to max of 300 mg daily; maintenance dose: 4-8 mg/kg in divided doses. Tonic-clonic status epilepticus 10-15 mg/kg by slow IV or intermittent infusion not >50 mg/min, max: 50 mg/min.
Special Precautions
Impaired liver function. Diabetic patients. Avoid extravasation & intra-arterial administration. Patients w/ sinus bradycardia, heart block or Stokes-Adams syndrome; hypotension, heart failure & MI. Monitor BP & ECG during IV therapy. Leucopenia. Discontinue immediately or permanently if rash develops. May interfere w/ some thyroid function tests. Pregnancy. Neonates. Elderly.
Adverse Reactions
Lack of appetite, headache, dizziness, transient nervousness, insomnia, GI disturbances eg nausea, vomiting & constipation. Tenderness & hyperplasia of the gums. Acne, hirsutism & coarsening of the facial features. Mild hypersensitivity reactions.
Drug Interactions
Antibacterials, anticoagulants, corticosteroids, quinidine, sex hormones, dopamine & lignocaine.
