Felodipine
Dosage/Direction for Use
Initially 5 mg once daily, may be increased to 10 mg once daily after at least 2 wk. Max: 10 mg. Elderly & patients w/ mild to moderate liver impairment Initially 2.5 mg once daily.
Administration
May be taken with or without food: Swallow whole, do not chew/crush. Avoid taking w/ grapefruit juice.
Contraindications
Hypersensitivity. Stroke w/in last 6 mth; CV shock; valvular heart disease (higher-grade aortic or mitral stenosis); heart muscle disease w/ narrowing of cardiac cavity (hypertrophic obstructive cardiomyopathy); unstable angina pectoris; acute MI (w/in last 8 wk); higher-grade disturbances of impulse conduction from atria to ventricles of heart (2nd- & 3rd-degree AV block); CHF. Severe impairment of kidney (renal insufficiency GFR <30 mL/min, creatinine >1.8 mg/dL) & liver function. Pregnancy.
Special Precautions
Conduction disorders, heart failure, tachycardia & haemodynamically relevant aortic &/or mitral valve stenosis. Avoid abrupt discontinuation as hypertensive crisis may occur. Mild to moderate hepatic impairment. May impair ability to drive or use machines. Lactation. Not to be used in childn.
Adverse Reactions
Flushing, headache or tinnitus.
Drug Interactions
Increased BP lowering effect w/ other BP lowering drugs & certain TCAs. Increased blood levels w/ cimetidine, erythromycin, itraconazole or ketoconazole. Increased plasma level w/ grapefruit. Reduced blood levels w/ carbamazepine, phenytoin or barbiturates.
