Enoxaparin Na
Prophylaxis of DVT w/c may lead to pulmonary embolism. Prophylaxis of ischemic complications of unstable angina & non-Q-wave MI when currently administered w/ aspirin. Prevention of thrombus formation in extracorporeal circulation during hemodialysis.
Dosage/Direction for Use
Adult Abdominal surgery 40 mg once daily SC. Initial dose given 2 hr pre-op then for 7-10 days. Hip or knee replacement surgery 30 mg every 12 hr SC. Initial dose given 12-24 hr pre-op. For hip replacement, 12 (±3) hr pre-op. Continued for 3 wk at a dose of 40 mg once daily SC. Risk of thromboembolic complications during acute illness 40 mg once daily SC for 6-11 days. Unstable angina & non-Q-wave MI 1 mg/kg SC 12 hrly for 2-8 days. DVT w/ or w/o pulmonary embolism 1 mg/kg 12 hrly. Prevention of thrombus formation in extracorporeal circulation during hemodialysis 1 mg/kg. Patient at high-risk of hemorrhage 0.5 mg/kg. Patient w/ double vascular access 0.75 mg/kg.
Patients w/ active major bleeding or w/ thrombocytopenia. Childn.
Special Precautions
Hepatic & renal impairment. Not for IM inj. Pregnancy & lactation. Patients w/ bleeding diathesis, uncontrolled arterial HTN, history of recent GI ulceration, diabetic retinopathy & hemorrhage. Perform periodic complete blood count. Elderly.
Adverse Reactions
Thrombocytopenia. Hematoma, intrahepatic hemorrhage, lumbar plexopathy, increased liver function tests, hypersensitivity, local reactions at the inj site.
Drug Interactions
Anticoagulants, platelet inhibitors, NSAIDs, dipyridamole, sulfinpyrazone, dextran 40, glucocorticoids.
