Indications/Uses
Susceptible infections eg, bone & joint infections, bronchitis (& other lower resp tract infections), gonorrhea, meningitis, otitis media, peritonitis, pharyngitis, sinusitis, skin infections (including soft-tissue infections), surgical infection (prophylaxis) & UTI.
Dosage/Direction for Use
Medzyme FC tab Adult 500 mg bid. Medzyme granules for oral susp Childn >2 yr 250 mg bid. Max: 500 mg daily, >6 mth 125 mg bid. Max 250 mg daily. Medzyme Vial Adult Severe infections 1.5 g tid or qid. Total dose: 3-6 g daily. Childn & infant 30-100 mg/kg/day as 3 or 4 divided doses. Neonate 30-100 mg/kg/day as 2 or 3 divided doses. Gonorrhea 1.5 g as a single dose. Prophylaxis: Abdominal, pelvic & orthopedic operations 1.5 g IV w/ induction of anesth, may be supplemented w/ two 750 mg dose IM 8 & 16 hr later. Cardiac, pulmonary esophageal & vascular operations 1.5 g IV w/ induction of anesth continuing w/ 750 mg IM tid for a further 24-48 hr. Total joint replacement 1.5 g mixed dry w/ methyl methacrylate cement. Meningitis Adult 3 g IV every 8 hr. Childn & infant 200-240 mg/kg/day IV in 3 or 4 divided doses. May be reduced to 100 mg/kg/day IV after 3 days or when clinical improvement occurs. Neonate Initially 100 mg/kg/day IV. May be reduce to 50 mg/kg/day when clinically indicated.
Administration
Should be taken with food.
Contraindications
Hypersensitivity to cephalosporins & penicillins.
Special Precautions
GI disease, particularly colitis. Pregnancy & lactation. Medzyme Vial Patients w/ transient or persistent renal insufficiency. Overgrowth of non-susceptible organisms during prolonged use. Interrupt treatment if severe superinfection occurs. Pseudomembranous colitis.
Adverse Reactions
Medzyme Erythema multiforme, Stevens-Johnson syndrome, toxic epidermal necrolysis. Hypersensitivity reactions including skin rashes, urticaria, pruritus, interstitial nephritis, drug fever, & serum sickness. GI disturbances including diarrhea, nausea & vomiting. Pseudomembranous colitis. Headache. Medzyme Vial Local irritation & thrombophlebitis following IV inj, diarrhea, maculopapular rash.
Drug Interactions
Reduced renal clearance w/ probenecid. Medzyme Vial May increase renal disorder w/ furosemide or aminoglycoside antibiotics.
