Indications/Uses
Central diabetes insipidus, primary nocturnal enuresis & symptomatic treatment of nocturia associated w/ nocturnal polyuria in adults.
Dosage/Direction for Use
SL Central diabetes insipidus Adult & childn Initially 60 mcg tid. Daily dose: 120-720 mcg. Maintenance: 60-120 mcg tid. Primary nocturnal enuresis Initially 120 mcg at bedtime, may be increased up to 240 mcg. Nocturia Initially 60 mcg at bedtime, may be increased up to 120 mcg after 1 wk & 240 mcg by wkly dose escalations.
Contraindications
Hypersensitivity to desmopressin or other drugs containing fish gelatin. Habitual or psychogenic polydipsia (urine production >40 mL/kg/24 hr), known or suspected cardiac insufficiency & other conditions requiring diuretics, moderate to severe renal insufficiency (CrCl <50 mL/min), syndrome of inappropriate ADH secretion (SIADH), known hyponatremia. Elderly ≥65 yr.
Special Precautions
Patients w/ urgency/urge incontinence, organic causes for increased micturition frequency or nocturia (eg, benign prostate hyperplasia, UTI, bladder stones/tumors), polydipsia & poorly adjusted DM. Water retention &/or hyponatremia may occur w/o concomitant reduction of fluid intake. Monitor serum Na in concomitant use w/ drugs known to induce SIADH & NSAIDs. Discontinue use during acute intercurrent illness characterized by fluid &/or electrolyte imbalance (eg, systemic infections, fever, gastroenteritis). Pregnancy & lactation.
Adverse Reactions
Headache, abdominal pain, nausea, hyponatremia, dizziness, peripheral edema, micturition frequency, dry mouth, increased wt.
Drug Interactions
Additive antidiuretic effect w/ increased risk of fluid retention w/ drugs known to induce SIADH eg, TCAs, SSRI, chlorpromazine & carbamazepine. Concomitant use w/ NSAIDs. 3-fold increase in desmopressin plasma conc w/ loperamide. Decreased absorption w/ dimeticone; extent & rate of absorption w/ high-fat meal.
