Dosage/Direction for Use
Nuelin SR 125/Nuelin SR 250 Adult 250 mg every 12 hr. Childn 12-25 kg 125 mg bid, >25 kg 250 mg bid. Nuelin 26.7 mg/5 mL Adult 25 mL every 6 hr. Childn >2 yr 1 mL/kg every 6 hr. Max: 25 mL.

Overdosage
View Nuelin/Nuelin SR 125/Nuelin SR 250 overdosage for action to be taken in the event of an overdose.
Administration
Tab: Should be taken with food: Swallow whole, do not chew/crush. Syr: Should be taken on an empty stomach. : Take 1 hr before meals w/ a glass of water. Take w/ or immediately after meals to avoid GI irritation.
Contraindications
Hypersensitivity to theophylline or xanthines.

Special Precautions
Patients w/ reduced thyroid function, CHF, acute pulmonary edema, COPD, severe hypoxia, pneumonia, acute febrile episodes & during acute viral infection; liver impairment eg hepatic cirrhosis. Cardiac arrhythmias, CAD, unstable angina, cardiomyopathy & severe HTN; peptic ulcers or GERD. Monitor plasma levels. Smoking. Concomitant use w/ other xanthines or xanthine containing beverages. Debilitated patients & elderly >65 yr. Pregnancy & lactation.

Adverse Reactions
Gastric irritation, nausea, vomiting, anorexia, epigastric pain, peptic ulcer reactivation, GERD, haematemesis, tachycardia, palpitation, headache, CNS stimulation, reflex hyperexcitability, insomnia, tremor; diarrhoea, extrasystoles, flushing, hypotension, tachypnoea, potentiation of diuresis, albuminuria, haematuria, rash, hyperglycemia, hypokalaemia, alopecia, inappropriate ADH secretion (high dose).

Drug Interactions
Decreased hepatic clearance w/ cimetidine, allopurinol (high-dose), propranolol, macrolides (eg, erythromycin, clarithromycin), quinolones (eg, ciprofloxacin, enoxacin), alcohol, OCs, mexiletine, tacrine, thiabendazole, disulfiram, interferon-α & verapamil. Increased hepatic clearance w/ tobacco or marijuana smoking, phenobarbitone, phenytoin, carbamazepine & rifampicin. Reduced plasma levels w/ St. John's wort. Ventricular arrhythmias w/ halothane. May lower seizure threshold w/ ketamine. Enhanced renal clearance of lithium. Synergism w/ adrenaline & other sympathomimetic amines. Cardiac arrhythmias w/ β-adrenergic agonist. Ranitidine, diltiazem, nifedipine, INH, frusemide, flu vaccine & corticosteroids.
