Indications/Uses
Post menopausal osteoporosis; renal osteodystrophy in patients w/ chronic renal failure; post surgical hypoparathyroidism; idiopathic hypoparathyroidism; pseudohypoparathyroidism; vit D-dependent rickets.
Dosage/Direction for Use
Adult Postmenopausal osteoporosis 0.25 mcg bid. Renal osteodistrophy in dialysis patient Initially 0.25 mcg, may be increased by 0.25 mcg daily at 2-4 wk intervals. Childn 1-5 yr (w/ hypoparathyroidism) 0.25-0.75 mcg/day. Hypoparathyroidism & rickets 0.25 mcg daily in the morning, may be increased at 2-4 wk intervals.
Administration
May be taken with or without food: Take w/ meals to reduce GI discomfort.
Contraindications
Special Precautions
High dose may cause hypercalcemia & hypercalciuria. Monitor Ca serum level twice wkly. Dialysis patients. Hypercalcemia in restricted patients (eg, after surgery). Ectopic calcification in renal impairment. Continue oral phosphate therapy in vit D-resistant rickets (familial hypophosphatemia). Do not take other vit D prep to prevent hypervitaminosis. Avoid dehydration. Pregnancy & lactation. Childn (in dialysis).
Adverse Reactions
Immediate reactions: Weakness, dizziness, drowsiness, dry mouth, nausea, vomiting, constipation, muscular pain, bone pain & metallic taste. Delayed reactions: Polyuria, polydipsia, anorexia, wt loss, nocturia, conjunctivitis, pancreatitis, photophobia, rhinorrhea, hyperthermia, reduced libido, BUN elevation, albuminuria, hypercholesterolemia, increased SGOT & SGPT, ectopic calcification, HTN, cardiac arrhythmia & psychosis (rare).
Drug Interactions
Reduced absorption of lipid-soluble vit w/ cholestyramine. Inhibited absorption w/ vit D analogue. Avoid uncontrolled Ca intake in diet. Increased risk of hypercalcemia w/ thiazides. May increase cardiac arrhythmia in digitalis therapy. May increase metabolism & reduce plasma conc w/ enzyme-inducing drugs (eg, phenytoin & phenobarb). Hypermagnesemia w/ Mg-containing drugs (eg, antacids).
