Metformin HCl
Type 2 DM particularly in obese & juvenile diabetics in whom diet alone has failed as monotherapy. Adjunct to diet & exercise to improve glycemic control in patients w/ type II DM.
Dosage/Direction for Use
500 mg once daily w/ increments of 500 mg wkly. Max: 2,000 mg daily.
Should be taken with food: Swallow whole, do not chew/crush.
Hypersensitivity. Renal or hepatic failure, alcoholism, NIDDM w/ severe ketosis & acidosis, diabetic precoma & coma. Patients undergoing surgery, severe trauma, infections, COPD, CHD, cardiac failure, peripheral vascular disease, hypoglycemia. Pregnancy.
Special Precautions
Discontinue if lactic acidosis occurs. Lactation. Childn. Elderly.
Adverse Reactions
Nausea, diarrhea, gastric pain, constipation, vomiting, metallic taste; rash, pruritus, urticaria, erythema & flushing. Headache, dizziness; impaired GI absorption of vit B12 & folic acid on long-term use.
Drug Interactions
Phenprocoumon, hyperglycemic agents (eg, thiazides & corticosteroids), alcohol, furosemide, nifedipine & cationic drugs (eg, amiloride, digoxin, morphine, procainamide, quinidine, quinine, ranitidine, triamterene, trimethoprim, cimetidine & vancomycin). Reduced absorption w/ acarbose & guar gum.
