Cilostazol
Indications/Uses
Treatment of various ischemic symptoms eg, ulceration, pain & cold sensation due to chronic arterial occlusive diseases. Prevention of recurrence of cerebral infarction excluding cardiogenic cerebral embolism.
Dosage/Direction for Use
Adult Pletaal 100 mg bid. Pletaal SR 200 mg once daily.
Administration
Should be taken on an empty stomach: Take at least 30 min before or 2 hr after meals.
Contraindications
CHF. Hemophilia, increased capillary fragility, hemoptysis; intracranial, digestive & urinary tract & vitreous body hemorrhage. Hypersensitivity. Women of childbearing potential. Pregnancy.
Special Precautions
Monitor for any anginal symptoms eg, chest pain. Asymptomatic cerebral infarction. Concomitant use w/ anticoagulants, antiplatelet & thrombolytic drugs or prostaglandin E1 or its derivatives. Risk of menorrhagia during menstruation; bleeding tendency or predisposition; severe hepatic or renal impairment; coronary artery stenosis; DM or abnormal glucose tolerance; malignant HTN. Discontinue use or reduce dose if excessive increase in pulse rate is observed in patients w/ coronary artery stenosis. Long-term use in patients w/o CHF. Lactation. Childn. Elderly.
Adverse Reactions
Rash; palpitation, tachycardia, hot flushes; headache/dull headache, dizziness, insomnia, numbness; abdominal pain, nausea, vomiting, anorexia, diarrhea, heartburn & abdominal distention; SC hemorrhage; increased ALT, AST, alkaline phosphatase & lactate dehydrogenase; sweating, edema & chest pain.
Drug Interactions
Anticoagulants eg, warfarin, antiplatelets eg, aspirin & ticlopidine HCl, thrombolytics eg, urokinase & alteplase, prostaglandin E1 or its derivatives eg, alprostadil & limaprost alfadex; CYP3A4 inhibitors including macrolides eg, erythromycin, HIV PIs eg, ritonavir, azole antimycotics eg, itraconazole & miconazole, cimetidine, diltiazem HCl & grapefruit juice; CYP2C19 inhibitors eg, omeprazole.
