Haloperidol
Indications/Uses
Treatment of schizophrenia & control of tics & vocal utterances of Tourette's disorder.
Dosage/Direction for Use
Control of acutely agitated schizophrenic patient w/ moderate to very severe symptoms 2-5 mg IM every 4-8 hr depending on the response, may be administered as often as every hr. Max: 20 mg/day.
Contraindications
Hypersensitivity. Severe toxic CNS depression/comatose, parkinson's.
Special Precautions
Dementia-related psychosis especially on elderly, not for IV administration. Monitor ECG for QT-prolongation & arrhythmias. Tardive dyskinesia, neuroleptic malignant syndrome. Patient w/ QT-prolongation conditions eg, long QT syndrome, hypokalemia, electrolyte imbalance. Pregnancy & lactation. Childn & elderly.
Adverse Reactions
Inj site reaction; tachycardia, hypotension, HTN, QT-prolongation &/or ventricular arrhythmia, Torsades de Pointes; extrapyramidal disorder, hyperkinesia, headache, tardive dyskinesia, oculogyric crisis, dystonia, dyskinesia, akathisia, bradykinesia, hypokinesia, hypertonia, somnolence, masked facies, tremor, dizziness; contipation, dry mouth, salivary hypersecretion, nausea, vomiting; rash.
Drug Interactions
Caution when in concomitant w/ medications known to prolong the QT-interval or known to cause electrolyte imbalance. If concomitant w/ antiparkinsonism medication, it may be continued after haloperidol is discontinued due to differences in excretion rates. Potentiates CNS depressants eg, anesth, opiates & alcohol. Increased QTc w/ metabolic inhibitors ketoconazole (400 mg/day) & paroxetine (20 mg/day). Increased conc w/ CYP3A4 or CYP2D6 inhibitors eg, itraconazole, nefazodone, buspirone, venlafaxine, alprazolam, fluvoxamine, quinidine, fluoxetine, sertraline, chlorpromazine, & promethazine. Reduced plasma levels w/ enzyme-inducing drugs eg, rifampin or carbamazepine.
