Cefuroxime axetil
Indications/Uses
Treatment of susceptible infections including upper & lower resp tract, uncomplicated skin & skin structure infections, uncomplicated UTI, uncomplicated gonorrhea & early Lyme disease.
Dosage/Direction for Use
Adult & adolescent ≥13 yr Acute sinusitis, tonsillopharyngitis & bacterial exacerbations of chronic bronchitis; uncomplicated skin & skin structure infections 250-500 mg 12 hrly for 10 days. Acute otitis media 500 mg 12 hrly for 10 days. Secondary bacterial infections of acute bronchitis 250-500 mg 12 hrly for 5-10 days. Uncomplicated UTI including pyelonephritis 250-500 mg 12 hrly for 7-10 days. Uncomplicated gonorrhea 1,000 mg as a single dose. Early Lyme disease 500 mg 12 hrly for 20 days.
Administration
Should be taken with food.
Special Precautions
Hypersensitivity to penicillins. Consider diagnosis of Clostridium difficile-associated diarrhea (CDAD) in patients who develop diarrhea. Discontinue use if seizure develops. History of GI disease particularly colitis. Monitor prothrombin time. Transient Jarisch-Herxheimer reaction. Porphyria; renal & hepatic impairment. Overgrowth of nonsusceptible organism (prolonged or repeated use). May impair ability to drive or operate machinery. Pregnancy & lactation. Elderly.
Adverse Reactions
Dermatologic/hypersensitivity reactions, GI, hematologic, nervous system, musculoskeletal, genitourinary, hepatic, resp & CV effects; Jarisch-Herxheimer reaction, mild to severe hearing loss, increased or decreased serum glucose conc, lockjaw-type reactions & viral illness.
Drug Interactions
Increased conc & reduced tubular secretion w/ probenecid. Increased risk of nephrotoxicity w/ aminoglycosides. Concomitant use of potent diuretics including furosemide & ethacrynic acid may increase the risk of renal toxicity. Decreased bioavailability w/ oral antacids. May reduce estrogen absorption & efficacy of combined OCs.
