Indications/Uses
Correction of metabolic acidosis associated w/ cardiac arrest in patients w/ preexisting acidosis; cardiac arrest associated w/ hyperkalemia w/ preexisting metabolic acidosis & life-threatening hyperkalemia w/ preexisting metabolic acidosis in adults, childn & neonates.
Dosage/Direction for Use
IV Adult & elderly 1 mmol/kg (1 mL/kg 8.4% soln) followed by 0.5 mmol/kg (0.5 mL/kg 8.4% soln) at 10 min intervals by slow IV inj. Childn 1 mmol/kg (1 mL/kg 8.4% soln) by slow IV inj.
Contraindications
Renal failure, metabolic or resp alkalosis, HTN, oedema, CHF, history of urinary calculi & coexistent K-depletion or hypocalcemia, hypoventilation, chloride-depletion or hypernatremia.
Special Precautions
Perform arterial blood gas analyses (arterial/venous blood pH & carbon dioxide levels) prior to & during the course of treatment. Avoid use of scalp veins. Pregnancy & lactation.
Adverse Reactions
Alkalosis, hypokalemia, hypernatremia, hyperosmolarity, hypocalcemia, hypoglycemia, paradoxical intracellular acidosis; deterioration of hemodynamic status associated w/ vol overload; intracranial hemorrhage (in neonate), hyperirritability or tetany; extravasation.
Drug Interactions
Corticosteroids or corticotrophin. May increase renal clearance of acidic products eg, tetracyclines especially doxycycline, acetylsalicylic acid, chlorpropramide, lithium, methenamine & increase t½ & duration of action of basic drugs eg, quinidine, amphetamines, ephedrine, pseudoephedrine, memantine & flecainide. May increase renal tubular reabsorption of mecamylamine. Hypochloraemic alkalosis may occur w/ K-depleting diuretics eg, bumetanide, ethacrynic acid, furosemide & thiazides. May reduce serum K conc of K supplements.
