Buprenorphine
Indications/Uses
Moderate to severe cancer pain & severe pain which does not respond to non-opioid analgesics.
Dosage/Direction for Use
Adult >18 yr Initial: 20 mg. Dose titration & maintenance therapy: Replaced after 96 hr or twice a wk at regular intervals.
Contraindications
Hypersensitivity. Opioid-dependent patients & narcotic w/drawal treatment. Severely impaired resp center & function. Patients receiving MAOIs or have taken w/in the last 2 wk. Myasthenia gravis; delirium tremens. Pregnancy.
Special Precautions
Acute alcohol intoxication, convulsive disorders, patients w/ head injury, shock, reduced level of consciousness of uncertain origin, increased intracranial pressure. Impaired resp function or those receiving medicinal products which can cause resp depression. Monitor patient w/ liver function disorders. Increased possibility of opioid reactions in febrile patients or w/ increased skin temperature. May impair ability to drive & operate machinery. Lactation. Childn <18 yr.
Adverse Reactions
Nausea, vomiting; erythema & pruritus.
Drug Interactions
MAOIs, other opioids, anesth, hypnotics, sedatives, antidepressants, neuroleptics, medicinal products that depress respiration & CNS. Alcohol. Intensified or weakened efficacy w/ CYP3A4 inhibitors & inducers.
