Cefixime
Indications/Uses
Treatment of infections caused by gm +ve & gm -ve microorganisms including bronchitis, bronchiectasis w/ infection, secondary infections of chronic resp tract diseases, pneumonia. Pyelonephritis, cystitis, cystourethritis, uncomplicated gonorrhea. Cholecystitis, cholangitis, scarlet fever, otitis media, sinusitis, tonsilitis, pharyngitis, typhoid fever.
Dosage/Direction for Use
Childn Total daily dose: 8 mg/kg as a single dose or in 2 equally divided doses 12 hrly except for UTI given as once daily dose. Susp Adult & childn >12 yr 20 mL once daily or 10 mL 12 hrly. Childn 7-12 yr 10-15 mL once daily or 5-7.5 mL 12 hrly; 3-6 yr 5-10 mL once daily or 2.5-5 mL 12 hrly; 1-2 yr 2.5-5 mL once daily or 1.25-2.5 mL 12 hrly. Drops Childn 6 mth-2 yr 2.5-5 mL once daily or 1.25-2.5 mL 12 hrly. Duration of treatment: 7-14 days. Infections due to Strep pyogenes At least 10 days. Typhoid fever Childn Total daily dose: 15-20 mg/kg as a single dose or in 2 equally divided doses 12 hrly for 7-14 days. Max: 400 mg/day.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to other cephalosporins or penicillins.
Special Precautions
Allergy to any drug. Consider Clostridium difficile-associated diarrhea & colitis in patients w/ diarrhea. History of colitis, GI malabsorption. Monitor prothrombin time in patients w/ renal or hepatic impairment, poor nutritional state, patients receiving protracted antimicrobial therapy & previously stabilized on anticoagulant therapy. Discontinue use if seizures occur. Avoid long-term or repeated use. False +ve reaction in nitroprusside test, Coombs' test, Clinitest, Benedict's & Fehling's soln. Pregnancy & lactation. Childn <6 mth.
Adverse Reactions
CNS & GI disorders; hypersensitivity reactions; hepatic, renal & hematologic effects. Dysuria, pyuria, genital pruritus, vaginitis, vag candidiasis; hyperbilirubinemia; increased serum amylase conc.
Drug Interactions
Carbamazepine, chloramphenicol, nifedipine, probenecid, salicylates (aspirin), warfarin & other anticoagulants.
