Indications/Uses
Treatment & prevention of bronchospasm in bronchial asthma, bronchitis, emphysema.
Dosage/Direction for Use
Tab Adult 4 mg (2 tab). If adequate bronchodilation is not obtained, may gradually increased dose to 8 mg (4 tab). Childn >12 yr 2-4 mg (1-2 tab), 6-12 yr 1 tab, 2-6 yr ½-1 tab. Elderly 1 tab. All doses to be taken tid-qid. Syr Adult 10 mL, dose may be gradually increased to 20 mL. Childn >12 yr 5-10 mL, 6-12 yr & Elderly 5 mL, 2-6 yr 2.5-5 mL. All doses to be taken tid-qid. Rotacap For inhalation w/ a Rotahaler. Relief of acute bronchospasm Adult 200 or 400 mcg. Childn 200 mcg. Prevention of allergen or exercise-induced bronchospasm Adult 400 mcg before exertion. Childn 200 mcg before exertion. Chronic therapy Adult 400 mcg tid-qid. Childn 200 mcg tid-qid. MDI Relief of acute bronchospasm Adult 100 or 200 mcg. Childn 100 mcg. May be increased to 200 mcg if required. Prevention of allergen or exercise-induced bronchospasm Adult 200 mcg before exertion. Childn 100 mcg before exertion. May be increased to 200 mcg if required. Chronic therapy Adult & childn Up to 200 mcg qid. Nebules Adult & childn Initially 2.5 mg by wet inhalation, may be increased to 5 mg & may be repeated qid. Severe airway obstruction Adult 40 mg/day w/ strict medical supervision.
Administration
Should be taken on an empty stomach: Take 1 hr before or 2 hr after meals.
Contraindications
Hypersensitivity. Threatened abortion. Rotacap: Patients w/ severe milk-protein allergy or have a history of salbutamol hypersensitivity.
Special Precautions
Inhaled salbutamol prep is not appropriate for managing premature labour. Sudden & progressive deterioration in asthma control. Thyrotoxicosis. Severe/underlying heart disease, assess CV status. Hypoxic patient on infusion, supplemental oxygen is required. May induce reversible metabolic changes in diabetic patient. Pregnancy & lactation. Syr: Increased risk of dental caries in long-term treatment. Nebules: Risk of anoxia through hypoventilation. Must be used by inhalation, w/ a nebuliser & should be breathe through the mouth. Must not be inj or swallowed. Patients known to receive large doses of other sympathomimetic drugs; thyrotoxicosis. Transient hypoxemia. Concomitant use w/ nebulized anticholinergics (ipratropium Br). Avoid contact w/ eyes. Increased blood sugar levels & ketoacidosis may develop in diabetic patients. Monitor for the development of elevated serum lactate & consequent metabolic acidosis. Infant <18 mth.
Adverse Reactions
Tremor, headache; tachycardia, palpitations; muscle cramps.
Drug Interactions
Hypersensitivity. Nonselective β-blockers eg, propranolol.
