Hypersensitivity to amlodipine or dihydropyridine derivatives & losartan, other angiotensin receptor blockers or dihydropyridine Ca channel antagonist. History of angioedema. Losartan: Concomitant use w/ aliskiren in patients w/ DM or renal impairment (GFR <60 mL/min/1.73 m2). Severe hepatic impairment. Pregnancy (2nd & 3rd trimester). Amlodipine: Severe hypotension, shock (including cardiogenic shock), obstruction of the outflow tract of the left ventricle (eg, high grade aortic stenosis), haemodynamically unstable heart failure after acute MI.
Patients w/ intravascular vol-depletion (eg, those treated w/ diuretics), severe obstructive CAD, CHF; electrolyte imbalance; whose renal function depend on the activity of RAAS. Severe renal disease, unilateral or bilateral renal artery stenosis; hepatic impairment. Discontinue during pregnancy & lactation. Childn.
Losartan: Decreased conc w/ rifampin. Decreased active metabolite conc & increased conc w/ fluconazole. Increased serum K w/ K-sparing diuretics, K supplements or salt substitutes containing K. May reduce excretion of lithium. May diminish antihypertensive effect w/ NSAIDs including selective COX-2 inhibitors.
