Cefpirome sulfate
Indications/Uses
Infections due to susceptible organisms including UTI, resp tract, & skin infections; septicaemia & infections in immunocompromised patients.
Dosage/Direction for Use
1-2 g every 12 hr as IV inj over 3-5 min or infusion over 20-30 min. Renal impairment CrCl 20-50 mL/min 0.5 or 1 g bid, 5-20 mL/min 0.5 or 1 g once daily, ≤5 mL/min (in haemodialysis patients) 0.5 or 1 g once daily plus half-dose after dialysis section.
Contraindications
Hypersensitivity to cefpirome sulfate & other cephalosporins.
Special Precautions
Hypersensitivity to penicillins, history of allergy. Monitor renal & haematological status during prolonged & high-dose therapy. May interfere in creatinine conc measurement w/ Jaffe method & may produce falsely high values. +ve results for direct Coombs' test. False +ve reactions for glucose using copper-reduction reactions. Prolonged use may result in overgrowth of non-susceptible organisms. Renal impairment.
Adverse Reactions
Hypersensitivity reactions including skin rashes, urticaria, eosinophilia, fever, serum sickness-like reactions, & anaphylaxis. Neutropenia & thrombocytopenia. Transient increases in liver enzyme values. Convulsions & other signs of CNS toxicity (high doses). Pseudomembranous colitis (prolonged use). Pain at inj site (IM use) & thrombophlebitis (IV infusion).
Drug Interactions
Reduced renal clearance w/ probenecid.
