Cefuroxime Na
Infections due to susceptible organisms eg, otitis media, orbital cellulitis, urinary tract, skin & soft tissue, bone & joint infections; post-splenectomy sepsis of unclear etiology.
Dosage/Direction for Use
Adult 2-4 g daily divided to 9 hr IV push over 3-5 min or IV infusion over 15-30 min. Childn 100-150 mg/kg daily divided to 8 hr IV push over 3-5 min or IV infusion over 15-30 min. Max: 6 g/day. Newborn 50-100 mg/kg daily divided to 12 hr IV push over 5 min or IV infusion over 15-30 min. Gonorrhea 1.5 g as single dose (2 x 750 mg inj IM w/ different sites). Meningitis Adult 3 g IV every 8 hr. Childn & infant 150-250 mg/kg/day IV in 3 or 4 divided doses. Neonate 100 mg/kg/day IV. Prophylaxis 1.5 g IV (w/ induction of anesth for abdominal, pelvic & orthopedic operations) supplemented w/ two 750-mg IM doses 8 & 16 hr later. Cardiac, pulmonary, esophageal & vascular operations 1.5 g IV w/ induction of anesth, continuing w/ 750 mg IM tid for a further 24-48 hr. Total joint replacement 1.5 g may be mixed dry w/ each pack of methyl methacrylate cement polymer before adding the liqd monomer. Renal impairment CrCl 10-20 mL/min 750 mg bid, <10 mL/min 750 mg once daily. Patient on hemodialysis Further 750 mg dose given IV/IM at the end of each dialysis. Incorporation into peritoneal dialysis fluid 250 mg for every 2 L of dialysis fluid given IV. Renal failure patient on continuous arteriovenous hemodialysis or high-flux hemofiltration in intensive therapy units 750 mg bid.
Hypersensitivity to cephalosporins.
Special Precautions
Allergic reaction to penicillins or other β-lactams. Concurrent treatment w/ potent diuretics eg, furosemide or aminoglycosides. Monitor renal function as well as w/ the elderly & w/ preexisting renal impairment. Pregnancy & lactation.
Adverse Reactions
GI disturbances eg, dizziness, headache, fatigue, fever. Transient neutropenia, eosinophilia, hypoprothrombinemia, platelet dysfunction, hemolytic anemia. Transient elevation of liver enzymes.
Drug Interactions
Slight interference w/ copper reduction methods (Benedict's, Fehling's, Clinitest).
