Exclude malignancy in the presence of significant unintentional wt loss, recurrent vomiting, dysphagia, haematemesis, anaemia or malaena & when gastric ulcer is suspected or present. Monitor liver enzymes regularly in patients w/ severe liver impairment particularly on long-term use. Increased risk of Clostridium difficile-associated diarrheoa & osteoporosis-related fractures of the hip, wrist or spine. Monitor Mg levels prior to initiation of PPI treatment & periodically. Symptomatic & asymptomatic hypomagnesaemia. Concomitant w/ methotrexate. Co-administration w/ HIV PIs is not recommended. May reduce vit B12 absorption. May affect ability to drive or operate machinery. Do not use during pregnancy. Lactation.
May reduce or increase absorption of pH-dependent drugs (eg, ketoconazole). Reduced bioavailability of HIV PIs eg, atazanavir, nelfinavir. Coumarin anticoagulants eg, phenprocoumon or warfarin (monitor prothrombin time/INR). May elevate & prolong serum levels of methotrexate &/or its metabolite hydroxymethotrexate. Increased systemic exposure w/ CYP2C19 inhibitors (eg, tacrolimus & fluvoxamine).
