Lanthanum carbonate hydrate
Hyperphosphataemia in adults w/ chronic renal failure on haemodialysis or continuous ambulatory peritoneal dialysis.
Dosage/Direction for Use
Adult & elderly >65 yr Pre-treatment: Serum phosphate level >2.9 mmol/L Initial daily dose: 2,250 mg. >2.4 & ≤2.9 mmol/L Initial daily dose: 1,500 mg. >1.8 & ≤2.4 mmol/L Initial daily dose: 750 mg. Control of serum phosphate level From 750 mg, w/ most patients achieving acceptable levels at 1,500-3,000 mg/day. Monitor serum phosphate levels & titrate dose every 2-3 wk until an acceptable serum phosphate level is reached, w/ regular monitoring thereafter.
Should be taken with food: Take w/ or immediately after meals, w/ the daily dose divided between meals, eg, tds. Chew thoroughly before swallowing. Do not swallow whole.
Hypersensitivity. Hypophosphataemia.
Special Precautions
Tissue deposition of lanthanum particularly in bone, liver & stomach wall. Monitor serum Ca levels in patients w/ renal insufficiency. Monitor liver function tests. Patients predisposed to GI obstruction, ileus, subileus & perforation eg, those w/ altered GI anatomy (eg, diverticular disease, peritonitis, history of GI surgery, cancer & ulceration), hypomotility disorders (eg, constipation, diabetic gastroparesis), & those who concomitantly use medications known to potentiate these effects. Re-evaluate patients who develop severe constipation or other severe GI signs & symptoms. Patients w/ acute peptic ulcer, ulcerative colitis, Crohn's disease or bowel obstruction. Abdominal x-rays may have a radio-opaque appearance typical of an imaging agent. Monitor patients w/ hepatic impairment. Pregnancy. Discontinue breastfeeding during therapy. Childn <18 yr.
Adverse Reactions
Headache; abdominal pain, diarrhoea, nausea, vomiting. Hypocalcaemia; constipation, dyspepsia, flatulence.
Drug Interactions
Not be taken w/in 2-hr of dosing w/ Fosrenol are compd known to interact w/ antacids (eg, chloroquine, hydroxychloroquine, ketoconazole); tetracycline & doxycycline. Decreased oral ciprofloxacin bioavailability; take oral floxacin formulations at least 2 hr before or 4 hr after Fosrenol. Reduced absorption of levothyroxine; thyroid hormone replacement therapy should not be taken w/in 2 hr of Fosrenol dosing & closely monitor TSH levels.
