Ifosfamide
Indications/Uses
Inoperable malignant tumours that are sensitive to ifosfamide eg bronchial carcinoma, ovarian carcinoma, testicular tumours, soft-tissue sarcoma, breast cancer, pancreatic carcinoma, hypernephroma, endometrial carcinoma, malignant lymphomas.
Dosage/Direction for Use
In case of fractionated application (short infusion), a total dose of 250-300 mg/kg/course should be aimed at. Usual schedule: 50-60 mg/kg IV daily for 5 consecutive days. Therapy-resistant cases: 80 mg/kg daily for 2-3 consecutive days. Also see lit.
Contraindications
Severe bone marrow depression; impaired renal function; bilateral urinary obstruction, acute infections, acute hemorrhagic cystitis. Pregnancy.
Special Precautions
Patients of reproductive age should use contraceptives during therapy & for the following 6 mth. Unilateral nephrectomies; brain metastases, infection, electrolyte imbalance.
Adverse Reactions
GI upsets; alopecia; reduction in blood count; cystitis; reversible encephalopathy. Immunosuppression. Occasionally, renal, hepatic & cardiac dysfunction in rare cases. Risk of secondary malignancies.
Drug Interactions
Neurotoxicity, hematotoxicity & nephrotoxicity may be increased by prior cisplatin therapy. Increased hypoglycemic effects w/ sulfonylureas. Increased bone marrow depression w/ allopurinol. Disturbances of anticoagulant control of warfarin.
