Per Hyzaar tab Losartan K 50 mg, hydrochlorothiazide 12.5 mg. Per Hyzaar Plus tab Losartan K 100 mg, hydrochlorothiazide 12.5 mg. Per Hyzaar Forte tab Losartan K 100 mg, hydrochlorothiazide 25 mg
HTN in patients whom combination therapy is appropriate. Reduction in the risk of stroke in hypertensive patients w/ left ventricular hypertrophy.
Dosage/Direction for Use
HTN 1 Hyzaar tab once daily. Max: 2 Hyzaar tab or 1 Hyzaar Forte tab once daily. 1 Hyzaar Plus tab for patients titrated to Cozaar 100 mg who require additional BP control. Reduction in the risk of stroke in hypertensive patient w/ left ventricular hypertrophy Initially losartan 50 mg once daily, then add hydrochlorothiazide 12.5 mg or Hyzaar tab if inadequate. If needed, increased to losartan 100 mg & hydrochlorothiazide 12.5 mg or Hyzaar Plus, or if necessary, increase hydrochlorothiazide to 25 mg or Hyzaar Forte once daily.
May be taken with or without food.
Hypersensitivity to losartan K, hydrochlorothiazide or other sulfonamide-derived drugs. Anuria. Concomitant use w/ aliskiren-containing products in patients w/ DM or renal impairment (GFR <60 mL/min/1.73 m2).
Special Precautions
Severe renal (CrCl ≤30 mL/min) or hepatic impairment; angioedema. Do not initiate in intravascularly vol-depleted patients (eg, treated w/ high-dose diuretics). Not to be used as initial dose in elderly patients (Hyzaar Plus/Hyzaar Forte only). Pregnancy & lactation. Childn. Losartan: Bilateral renal artery stenosis or artery stenosis to a solitary kidney. Concomitant use of other drugs that may increase serum K. Not recommended to be used concomitantly w/ ACE inhibitors or aliskiren. Not for concomitant use w/ ACE inhibitors in patients w/ diabetic nephropathy. Hydrochlorothiazide: Monitor for hypotension & electrolyte/fluid imbalance eg, vol depletion, hyponatremia, hypochloremic alkalosis, hypomagnesemia or hypokalemia. Perform periodic determination of serum electrolytes. Dose adjustment of antidiabetic agents including insulin may be required. Discontinue thiazides before carrying out tests for parathyroid function. Increase in cholesterol & triglyceride levels. May precipitate hyperuricemia &/or gout in certain patients. Risk of non-melanoma skin cancer; reduce sun & artificial UVA exposure. Regularly check skin for new lesions & promptly report suspicious skin lesions. Hypersensitivity reactions w/ or w/o a history of allergy or bronchial asthma. Exacerbation or activation of SLE.
Adverse Reactions
Dizziness, asthenia/fatigue, vertigo.
Drug Interactions
Reduced excretion of lithium; effect of diuretic, natriuretic & other antihypertensive w/ NSAIDs including selective COX-2 inhibitors. Losartan: Rifampin & fluconazole. Increased serum potassium w/ K-sparing diuretics (eg, spironolactone, triamterene, amiloride), K supplements or salt substitutes containing K or other drugs that may increase serum K (eg, trimethoprim-containing products). Increased risk of hypotension, hyperkalaemia & decreased renal function in concomitant use w/ ACE inhibitors or aliskerin. Hydrochlorothiazide: Potentiate orthostatic hypotension w/ alcohol, barbiturates or narcotics. Antidiabetic drugs (oral agents, insulin), other antihypertensive drugs. Impaired absorption w/ cholestyramine & colestipol resins. Intensified electrolyte depletion w/ corticosteroids, ACTH, glycyrrhizin (liquorice). Possible decreased response to pressor amines (eg, adrenaline); increased response to nondepolarizing skeletal muscle relaxants (eg, tubocurarine). May interfere w/ parathyroid function test.
