Contents
Per 50/100 mcg accuhaler Salmeterol xinafoate 50 mcg, fluticasone propionate 100 mcg. Per 50/250 mcg accuhaler Salmeterol xinafoate 50 mcg, fluticasone propionate 250 mcg. Per 50/500 mcg accuhaler Salmeterol xinafoate 50 mcg, fluticasone propionate 500 mcg. Per 25/50 mcg evohaler Salmeterol xinafoate 25 mcg, fluticasone propionate 50 mcg. Indications/Uses
Regular treatment of asthma where use of a combination (bronchodilator & inhaled corticosteroid) has been found to be appropriate. Symptomatic treatment of patients w/ severe COPD [forced expiratory vol in 1 sec (FEV1) <50% predicted normal] & a history of repeated exacerbations who have significant symptoms despite regular bronchodilator therapy.
Dosage/Direction for Use
Accuhaler Asthma Adult & adolescent ≥12 yr 1 inhalation of 50/100, 50/250 or 50/500 bd. Childn ≥4 yr 1 inhalation of 50/100 bd. COPD Adult 1 inhalation of 50/500 bd. Evohaler Asthma Adult & adolescent ≥12 yr 2 inhalations of 25/50, 25/125 or 25/250 bd. Childn ≥4 yr 2 inhalations of 25/50 bd.
Contraindications
Special Precautions
Not for relief of acute asthma symptoms. Do not stop abruptly due to risk of exacerbation. Possible development of pneumonia in patients w/ COPD. Patients w/ active or quiescent pulmonary TB, thyrotoxicosis, preexisting CV disease, low levels of serum K. Systemic effects may occur at high doses for prolonged periods. Regular monitoring of height in childn on prolonged treatment & adrenocortical function. Possibility of impaired adrenal response in patients transferring from oral steroid therapy. History of DM. Concomitant use w/ ritonavir, ketoconazole & other potent CYP3A4 inhibitors. Discontinue use if paradoxical bronchospasm occurs. Pregnancy & lactation.
Adverse Reactions
Headache. Mouth & throat candidiasis, pneumonia (in COPD patients); hoarseness/dysphonia; muscle cramps, arthralgia.
Drug Interactions
Avoid nonselective & selective β-blockers, ritonavir, other potent CYP3A4 inhibitors. Increased plasma salmeterol exposure & prolongation of QTc interval w/ ketoconazole.
